New Type of Ankle-foot Orthosis in Hemiplegia: A Case Report

摘要

腦血管疾病的主要後遺症是導致個案肢體癱瘓，進而導致個案行走能力的困難。足踝裝具主要作用為矯正垂足、預防足部內翻及提升改善行走能力。本研究目的探討新型足踝裝具對半側偏癱個案在功能行走和平衡能力之影響。方法：本研究兩位個案為腦血管疾病，左側下肢肢體為布朗斯壯分期第三期，均有垂足及足部內翻之情形，個案在研究均穿戴過前置式足踝裝具。評估項目：六分鐘行走測驗、計時起立、行走測驗、伯格平衡量表及行走速度測驗，測試評估順序：裸足、前置式足踝裝具、新型足踝裝具三項之行走平衡方式。結果：本研究結果顯示有穿戴足踝裝具比未穿戴足踝裝具在各項測試有明顯差異，在穿戴足踝裝具行走速度測試項目中行走速率均達每秒0.24公尺以上，平衡能力方面兩種足踝皆無明顯的差異。結論：本研究個案穿戴新型足踝裝具三週後，與前置式足踝裝具比較，在行走耐力、功能性行走及行走速度有明顯的差異，而對於新型足踝裝具整體覺得非常滿意。

Hemiplegia is a common sequela observed in patients suffered from cerebrovascular disease, which often leads to difficulties in walking ability. The main role of ankle-foot orthosis is to adjust drop foot and to prevent foot inversion which helps to improving walking ability. This study is to explore the effects of the new type of ankle-foot orthosis in hemiplegia patients particularly in their walking and balance ability. Method: This paper focuses on two case of studies of cerebrovascular disease patients, who are at Brunnstron stage III in the lower left limb. Both of the patients show signs of drop foot and inversion and, in addition, they both have experiences in wearing anterior ankle-foot orthosis prior to the study. The assessments include Six-Minute Walk Test, Timed Up & Go test, Berg Balance Scale and gait speed test. The evaluation sequence were barefoot, anterior ankle-foot orthosis and new type of ankle-foot orthosis. Result: The result demonstrates significant differences in each assessment test when comparing patients with or without the ankle foot orthosis. In the ankle-foot orthosis-on test, the walking speed of the patients was recorded to reach 0.24 meters per second. However, there were no differences observed between new ankle-foot orthosis and anterior ankle-foot orthosis in balance test. Conclusion: In this study, there were differences in walking endurance, functional walking and walking speed after wearing new type of ankle-foot orthosis for three weeks, in comparison with anterior ankle-foot orthosis. In addition, both patients were satisfied with with new type of ankle-foot orthosis used in the study.
