摘要

本篇係探討一位因車禍導致頭部外傷，術後伴隨意識不清及部分神經功能受損之個案於恢復期間的護理經驗過程。筆者於民國91年2月19日至91年3月7日共16天擔任主護護理師，藉由與個案互動之語言與非語言行為，並由家屬提供相關之訊息，運用整體性之護理概念，評估個案的需求，確立期護理問題為吞嚥困難、身體活動功能障礙、言辭溝通障礙、營養狀況改變少於身體需要及照顧者角色緊張等。在照護期間筆者扮演連絡者、諮詢者、指導者與傾聽者的角色，協助個案家屬度過危機。使個案能在護理期間減輕生理的不適，並面對自己的疾病，最後可自我完成不分生理功能的照顧。出院後經電話訪談得知個案已開始工作。願以此成功經驗與護理同仁共同分享。

The paper is concerned with a post-operative brain injury patient who had neurological deficit. From 2002.02.19 to 2002.03.07, holistic nursing concept was applied. Through verbal and nonverbal communication, we found some nursing problems: 1.impaired of swallowing. 2.disturbance of physical mobility. 3.impaired of verbal communication. 4.less than body requirement of nutrition. 5.caregiver role strain. During this period, nurses palyed a role as a communicator, consultant, teacher and listener to help the patient and his family to face disease crisis. After discharge, the patient successfully returned to his work. I like to share this special nursing experience with other health care providers in order to improve quality of care for patient who suffer from brain injury.
