摘要

本報告係描述一位罹患心房中膈缺損而行心房中膈修補手術的青少年面臨手術之衝擊而致身心困擾之護理經驗、筆者於護理期間(94.1.21~94.2.1)運用戈登十一項功能性健康型態評估工具，以會談、觀察、身體評估、視覺疼痛類比量表等方法收集資料，發現個案主要護理問題爲焦慮、疼痛、潛在危險性感染、知識缺失等。在此期間，筆者以手術前後整體護理概念，藉由傾聽、陪伴、主動關懷、配合個案的需求，提供開心手術前後之相關照護，協同醫療團隊與家屬討論治療計劃，協助個案及家屬平安順利面對衝擊，並學習自我照護。期盼能做爲病房護理人員於日後照顧此類個案之參考,提升護理的品質。

This case report presents the nursing experience of a teenager with ASD facing physical and mental problems caused by the operation. Based on Gorden functional health assessment guide, the author assesses the patient s condition and collects data with observation, interview, VAS (Visual Analogue Scale) from Jan. 21 to Feb.1, 2005. Four nursing problems are found accordingly: anxiety, pain, risk of inflection and knowledge deficient. Within this period, the author uses the pre-operation and post-operation nursing concept, offering caring and satisfying needs of the patient by listening, company, caring. Further, through discussion on the care plan with the medical team and family, the author assists the patient and family to overcome the impact better and become acquainted with self-care. Finally, this report is expected to be the valuable reference and better enhance quality of comprehensive care for the nursing staff.
