減輕一位外籍配偶主要照顧者負荷過重的護理經驗

本篇報告係描述協助一位外籍配偶主要照顧者面臨主要關係人(案夫)腦中風時，所產生身、心、社會負荷之護理經驗。筆者於94年3月1日至94年3月20擔任案夫之主護護士，護理期間以個案管理之概念藉直接照顧案夫及觀察、會談、傾聽等技巧收集資料，發現個案主要護理問題有照顧者角色緊張、溝通障礙、家庭因應能力失調、社交隔離等。筆者運用眞誠、接納、不批判的態度，建立良好的護病關係及信任感，提供正碓護理資訊，鼓勵說出内心的感受，紓解樂張，並適時運用醫療團隊資源，使個案能勇敢自我面對問題，順利學習照顧技巧，進而正向面對漫長的照顧責任。

The purpose of this report is to present the nursing experience of a stroke patient and his primary caregiver, Focused on the physical, mental and social overload of primary caregiver. During the nursing period,the author is primary nurse of patient. Using the concept of management, Data was collected by clinical observation, interviewing and listening. through directing care of patient, Major health problems of the subject were found, such as caregiver role strain,impaired of communication, ineffective of family coping and social isolation. During the nursing process, The author not only used honest acceptance,no critical attitude but also build good relationship and trust with subject,to provide suitable nursing information assisting the subject to face herself problem,develop the caring skill and positively play the role of long-term caregiver.
