慢性雙極性情感障礙患者服藥不遵從之照護經驗

Nursing experience of medication non-adherence in the client with chronic bipolar disorder

摘要

本文描述一位慢性雙極性情感障礙患者長期不規則服藥，使症狀反覆發作而入院治療，筆者於2009年11月11日至2009年12月18日護理期間依據Watson人性化關懷理論評估身心社會靈性層面並歸納出健康問題含：睡眠型態紊亂、孤寂感、服藥認知不足、低自尊及無望感。在護理過程中運用Watson十項關懷照護因素與個案建立信任感，提供疾病相關資訊協助個案坦然面對，在家人的支持中重新尋找希望，協助個案與他人互動，肯定自我價值，進而願意規則服藥並主動參與自我照顧。藉由此次照護經驗，期望能提供護理人員在照護類似病人的參考與運用。

This article the experience of a nurse directly responsible for the care of a patient with chronic bipolar disorder who irregular long-term medication, so that repeated attacks of symptoms while hospitalized. The author collected data through Watson’s caring theory between November 11th, 2009 and December 18th, 2009. The aothor confirmed the diagnosis and found that the patient had some related mental problems, including sleep pattern disturbance, loneliness, low self-esteem, hopelessness. The author established a positive relationship with the patient utilizing Watson’s caring theory and case-establishment trust in the nursing process. The author provided disease-related information to the patient, worked as an intermediary to encourage greater family member support and encouragement, helped the patient interact with visitors and encouraged development of the patient’s self-esteem. This experience can benefit nursing professionals by fostering a greater understanding of patient self-esteem and the benefits of a positive treatment environment.
