髖關節擺位技術對骨質密度測量結果之影響

Effect of Hip Positioning on Hip Bone Mineral Density Measurement

摘要


隨著年紀增長,骨質會漸漸流失,易造成骨質疏鬆症,因此提高骨折發生的機率。而在骨質疏鬆症的診斷上,骨質密度檢查佔了相對重要的地位。然而,擺位不正確常常會造成骨質密度檢查數據的差異,本研究的目的,便在於評估此數據的差異,同時探討此差異對檢查結果準確性造成的影響。在本研究中,我們收集需實施髖關節的骨質密度檢查的病患100位的資料,平均年齡41到70歲,以正確的擺位,將腳內旋看不到小轉子做掃描,之後並與腳沒有刻意內旋姿勢掃描的結果,進行數據比較其差異性。結果發現沒有刻意向內旋轉的骨質密度與T值較高,從而降低檢查的敏感度。此結果顯示正確的姿勢在髖關節骨質密度檢查結果的重要性,有利提供醫師在診斷上,達到正確的評估。

Bone density gradually loses with increasing age leading to osteoporosis and increased risk of fracture. Bone mineral density (BMD) measurement is relatively important in diagnosis of osteoporosis. The result of BMD measurement is influenced by positioning. The aim of this study is to determine if there is significant difference in the result of BMD measured with optimal and suboptimal positioning. One hundred patients receiveing dual-energy X-ray absorptiometry (DXA) of the hips for BMD measurement were recruited, with the age ranging from 41 years to 70 years. DXA study was performed with optimal internal rotation (little or none of the lesser trochanters shown) and without internal rotation, and the results were compared. There were increased BMD and T-score in the study group without internal rotation, indicating lower sensitivity of the exam. The result shows that the importance of proper positioning on BMD measurement of the hip which provides the physician precise evaluation.
