Using the Stress Radiographic Device to Improve Imaging Quality of Ulnar Deviation Projection in Wrist

利用關節施壓照相輔助裝置提昇腕部尺側曲屈攝影檢查的影像品質

摘要


The scaphoid fractures are the most common carpal fracture which account for 70-80% of all carpal bone fractures. There is rarely bruising and no deformity when a scaphoid fracture occurs, which is why it may be mistaken for a sprain. There are serious complications such as nonunion and avascular necrosis if scaphoid fractures are not diagnosed and treated in time. In the past, imaging quality of wrist ulnar deviation projection was unsatisfied due to patient motion. By using the stress radiographic device to perform ulnar deviation projection, patients are more likely to keep in a steady position. With nearly 80% in our data showed that using stress radiographic device in wrist deviation projection can clearly identify scaphoid outline and can fully reveal the maximum of the joint cavity between scaphoid and the nearest carpal bone. These results demonstrated imaging quality of wrist ulnar deviation projection is improved by using the stress radiographic device.

舟狀骨骨折,是常見的腕骨傷害,常被誤診為扭傷而延遲治療時機,造成舟狀骨癒合不全或壞死及腕部關節炎。一般進行腕部尺側屈曲攝影中,無法給予病人適當儀器加以施壓及固定,僅以病患容忍程度內,自行曲屈腕部來完成攝影,使得病患姿勢穩固性較差,本研究運用原有的關節施壓照相輔助裝置,經由變更重組出適合腕部尺側屈曲攝影之擺設方法,並以病患容忍程度內加以固定及施壓來完成此攝影檢查。本方法不僅可以讓病患以穩固的姿勢來接受檢查,也能提昇影像完成率及臨床醫師診斷率。此次研究從2017年01月01日至2017年12月31日收集共100位門診來診之相關病例,不分男女。在經由醫師的評估與病患的同意下將其分成有使用輔助裝置與無使用輔助裝置各50位病患兩組來討論。數據結果顯示使用輔助裝置完成攝影檢查得到的影像,不論是舟狀骨輪廓完整性的辨別或是與鄰近腕骨關節腔間隙距離的拉開,都明顯優於傳統攝影,有近80%以上能清楚辨別舟狀骨輪廓並且能充分拉開與鄰近腕骨的距離,並且無年齡上的差異,都能供良好的影像診斷價值。
