Interventional Treatment for Coronary Artery Aneurysm Formation after CABG: A Case Report

The coronary artery aneurysm is a sporadic condition. The cause is unclear, but according to our hospital's experience, the incidence rate is between 0.3% and 0.5%. The treatment methods are still controversial, mainly based on conservative medication, surgical ligation, and covered stent deployment, each with its pros and cons. This case report concerns a 63-year-old man admitted to acute coronary syndrome with pulmonary edema two years after coronary artery bypass surgery. After cardiac catheterization, an aneurysm was found in the right coronary artery. Due to the fact that the patient's vital signs were unstable, his family decided to treat it with interventional therapy-the way to place the covered stent. During the treatment, intravascular ultrasound (IVUS) was applied, and the appropriate size of the covered stent was selected. The postoperative recovery of the patient was respectively good; currently, he is regularly followed up in the OPD.

冠狀動脈血管瘤是非常少見的狀況，發生原因並不明確，以本院的經驗來說發生率介於百分之0.3～0.5之間。而治療方式目前仍存在爭議，主要是以保守治療、外科手術與置放覆膜支架為主。本病例報告為一位63歲男性在冠狀動脈繞道手術後兩年因急性冠心症合併肺水腫入院治療，在心導管檢查後發現繞道手術所接的隱靜脈已完全阻塞，供應左前降支的左內乳動脈血流為TIMI III，右冠狀動脈中段有一個血管瘤，因病人生命徵象不穩定，家屬決定以介入治療的方式置放覆膜支架。治療過程中使用血管內超音波輔助，選擇合適的覆膜支架規格，術後病人的恢復的狀況良好，目前定期於門診追蹤。
