癌症免疫治療導致之皮膚副作用與治療

摘要

免疫治療是一種使用免疫檢查點抑制劑（immune check point inhibitor）治療癌症的新型療法。其原理在於透過抑制免疫檢查點，達到免疫細胞活性的增強，進而使免疫細胞有效辨識並消除人體內的腫瘤細胞。目前，免疫治療藥物大多針對Cytotoxic T-lymphocyte-associated protein 4（CTLA-4）、Programmed cell death protein 1（PD-1）、Programmed cell death protein ligand 1（PD-L1）等受器進行抑制。已經有許多證據指出，免疫治療能運用在晚期黑色素細胞癌、肺癌、泌尿上皮癌、與頭頸癌等癌症。免疫治療也在許多國外的醫學會（包含：美國、歐洲、日本）被列為治療準則之一，在臺灣也廣泛地被運用當中。許多新興的醫學研究和臨床試驗也正嘗試擴大免疫藥物在其他部位癌症的適應症，希望能為癌症病友提供更豐富的治療選項。然而，免疫治療並非完美的仙丹。就和其他藥物一樣，免疫治療的使用仍有可能產生相關的副作用。在這其中，最常見的即為皮膚相關的副作用。有鑒於此，本文整理了免疫治療導致之皮膚副作用，以及相對應的治療。希望本文的介紹能夠幫助大家更認識免疫治療，也能夠給予癌症病友充分的衛教，來早期發現自己身上可能出現的皮膚不良反應。
