Size Matter, Learning Curve and Oncological Outcome of Transoral Endoscopic Thyroidectomy via Vestibular Approach

Background: Transoral endoscopic thyroidectomy via the vestibular approach (TOETVA) is the latest natural orifice transluminal endoscopic surgery employed in thyroid surgery. The benefit of TOETVA is the potential for truly a "scarless" surgery that does not leave any cutaneous incision. TOETVA has become popular in both Asian and Western countries. The present review focus on the current clinical evidence of this innovative surgery about the issue of tumor size matter, learning curve, and oncological outcome. Methods: A literature review. Results: To overcome the anatomic obstacles for TOETVA, there were several modified techniques developed, including cut-down tumor size and hybrid approach (combined transoral and retroauricular or submental access) in order to extract large tumors. The learning curve of TOETVA for most research results is estimated to be 7 to 15 cases. The initial results of oncology, postoperative complications, and cosmetics supported the application of TOETVA in selected papillary thyroid carcinoma. Conclusions: The advantage of TOETVA is excellent cosmetic outcomes, shorter learning curve, and it can be considered safety and efficacy oncological outcomes in the present short period of studies.
