應用磁振擴散加權影像檢測肝細胞癌最佳化b值

Clinical Application of the Optimal b Value in Diffusion-Weighted MR Imaging for the Detection of Hepatocellar Carcinoma

摘要


磁振擴散加權影像技術(Diffusion Weighted MR Imaging, DWI)經後處理量化可測量表面擴散係數(Apparent Diffusion Coefficient, ADC),藉表面擴散係數的變化評價肝腫瘤病變,但這些腫瘤的表面擴散係數,涉及到不同擴散敏感度因子b值,應用五個不同b值(50,150,300,500,800)s/ mm^2檢測以肝臟良性與惡性腫瘤,經統計分析腫瘤ADC values、Likert 5分法進行效度,信度檢定、準確率及ROC curve,以確認磁振擴散加權影像技術最佳化b值,結果:隨著b values愈大,腫瘤ADC values愈小,DWI訊號減弱,五個不同b values皆對肝細胞癌、血管瘤、正常肝臟組織所得ADC values有顯著差異,p < 0.001(ANOVA)。不同b values的DWI其腫瘤受血管影響程度,以Likert 5分法計分,信度檢定Cronbach’s alpha值為0.841,隨著b values越大,腫瘤受血管影響程度越小,以b=500 s/mm^2與b=800 s/mm^2所得的評分結果相同為4.833± 0.049,影響程度最小。肝細胞癌b= 500 s/ mm^2,ROC curve下面積,95%信賴區間(AUC=0.747,p<0.001),血管瘤b=500 s/ mm^2,ROC curve下面積(AUC),95%信賴區間(AUC= 0.947,p<0.001) 結果顯示:為能偵測區分常見肝細胞癌與血管瘤,DWI設定b值為500s/ mm^2為最佳診斷工具。

We use ADC to evaluate hepatic tumors. The purpose of this study was to evaluate the performance of ADC(Apparent Diffusion Coefficient, ADC) under five different settings of b value (b=50,150,300,500 and 800 s/mm^2).Results: With higher b value, the ADC value of the tumors decreased, and tumoral signal intensity on DW images(Diffusion Weighted MR Imaging, DWI) decreased. All hepatomas, hemangiomas and adjacent liver parenchyma showed different ADC values under all five sets of b values (p<0.001). Tumoral signals on DW images were less affected by the presence of adjacent vascular structure on high b value images. Both b value=500 s/mm^2 and b value =800 s/mm^2 images showed best result when examed with five-point Likert scale. Under the setting of b value 500 s/mm2, AUC of hepatoma was 0.747, AUC of hemangioma was 0.947 (p<0.001).Conclusion: When using DWI to evaluate hepatic tumors, set the b value as 500 s/mm2 results in best diagnosis quality.
