微脂體包覆電腦斷層對比劑於誘發腎病變之預防

Liposome in Computed Tomography Contrast Medium induced Nephropathy Prevention

摘要


本研究探討以奈米載體(微脂體,liposome)包覆電腦斷層對比劑預防對比劑誘發腎病變(contrast induced nephropathy,CIN)之效果。將含碘對比劑包覆於微脂體內,活體外實驗比較在微脂體包覆與否的情形下,含碘對比劑對狗腎臟上皮細胞(MDCK cell)之毒性,將致毒劑量(20mg/ml)經與細胞共培養24 小時,發現微脂體包覆可大幅提升存活率(76%至94%);活體實驗以藥物streptozotocin (STZ)誘發糖尿病之大鼠為對象,比較在微脂體包覆與否的情形下,經尾靜脈注射含碘對比劑對腎臟功能影響之差異,經注射48 小時後分析血液肌酸酐(Creatinine)及尿素氮(BUN)之濃度變化為指標,注射未包覆對比劑之大鼠肌酸酐濃度提高了27%,尿素氮濃度也提高12.6%,而注射微脂體包覆對比劑之大鼠肌酸酐濃度下降7.4%,尿素氮濃度下降20%。綜合實驗結果,微脂體包覆對比劑能降低腎臟細胞的死亡率,同時,可以減少腎臟功能不良發生的機會並預防CIN 發生,改善傳統對比劑所帶來的副作用。

The aim of the study is to investigate the potential of liposome as computed tomography contrast agent delivery vesicle for contrast media induced nephropathy prevention. Iodinated contrast media were encapsulated in liposome. From in vitro study, liposome encapsulation was found to significantly improve the viability of canine renal epithelial cell (MDCK cell) from 76% to 94% after 24 hours co-cultured with toxic iodine dosage, 20mg / ml. For in vivo study, diabetic rats induced by streptozotocin (STZ) and administrated with iodinated agent with or without liposome encapsulation through tail vein. It was found that renal function indices, blood creatinine and BUN, were increased by 27% and 16% in control group without liposome encapsulation while they were reduced by 7.4% and 20% in experimental group with liposome encapsulation 48 hours after administration. The preliminary result indicated that liposome encapsulation has the potential to prevent the contrast media induced nephropathy of conventional contrast media.
