特殊營養食品暨膠囊錠狀食品審查機制及服務流程改善專案

Safety and Accessibility Review and Improvement of a Prescription Food System

摘要


本專案目的為指導建立特殊營養食品暨膠囊錠狀食品管理制度、建置食品類功能性之實證專業審查機制及改善食品開立及收費整體服務流程。本專案之特殊營養食品係指特定疾病配方食品、嬰兒與較大嬰兒配方食品;食品類則指非傳統食用形態的膠囊錠狀食品,包括膳食營養補充劑及健康食品。專案執行起訖為2016年10月至2020年12月,本專案改善策略規劃如下:一、建立特殊營養食品暨膠囊錠狀食品管理制度及食品類功能性之營養專業暨實證審查機制、二、簽訂完整食品類合約並標準化採購驗收流程、三、建置開立至收費服務流程連貫的資訊系統。專案執行期間之成效如下:在食品類審查中營養師透過實證醫學手法於產品功能性上進行營養專業暨實證審查,特殊營養食品及食品類案件審查共115件,其中特殊營養食品審查通過率89%,食品類則為51%。通過審查之產品,在2016年至2020年間完成簽訂合約,全數依照標準程序進行食品類採購驗收及倉儲管理作業。2017年6月完成建置醫師開立及營養師評估之資訊系統,系統連結院方批價收費流程。此外,病人端於安心購買面向、服務態度、價格及付款便利性上獲得92.5~100%的滿意度。在本院醫護同仁端於建立食品使用功能性審查制度之必要性、安全性及便利性獲得99.1~100%的正向認同。故專案改善後,本院已建置食品類管理制度,在專業審查、採購進用及開立收費,已建立一致性的標準作業程序,特殊營養食品審查通過率為89%,食品類通過率為51%。專案執行後,除了提升病人滿意度外,亦獲得醫護同仁的正向支持,顯示食品類管理在臨床使用之重要性。

The purpose of this quality improvement project was to rebuild a management system for foods for special medical purposes (FSMP) and tablet/capsule food supplements, by implementing a review mechanism and applying an evidence-based medicine (EBM) approach in the evaluation and by further improving patient accessibility. To improve our system, we aimed to (1) establish evidence-based review system to assure functionality of these food supplements, (2) develop a standardized contract and procurement operation, and (3) improve information technology in order to establish well-integrated and coherent prescription and charging process. Using an EBM approach to evaluate food supplementation, we found the pass rate of FSMP was 89%, and that of tablet/capsule food supplement was only 51%. Contract signing and the process of procurement were standardized and fully executed based on the operation protocols between 2016 and 2020. The information technology system was successfully established allowing physicians and dietitians to prescribe food supplements and it was rebuilt to be integrated with a friendly charging process. Based on a survey we administered, the patients' satisfaction rating of the attitude of the hospital staff and system accessibility ranged from 92.5% to 100%. Hospital staff also held positive views towards the necessity of rebuilding our food supplement management system to ensure food safety and improve accessibility of utilization by customers (99.1%-100%). Thus, after implementing this quality improvement project, we rebuilt our food supplements management system, established an EBM review system, and standardized the procurement process. High satisfaction rating from patients and positive view of our hospital staff were also found. Additionally, this project also brought attention to and emphasized the importance of reviewing the functionality of food supplements before clinical use.
