摘要
本研究著眼於人口老化議題，我國政府因為人口老化趨勢以及少子化的情形，從1981年起就發展出各種老人政策，包含最近通過的長期照顧十年計劃2.0，其主要目的就想達到在地老化的理想目標，本研究針對其推動方式中的一項，社區預防照顧，進行研究，該項推動方式之主要服務目的內容就是強調積極建構在地老化及健康老化的社區初級預防性照顧服務，以社區積極預防概念，建立社區照顧關懷據點及提升據點服務量能，本研究為讓這樣的推動方式更加順利與方便，因此設計開發一款APP可以輔助社區與居家老人之間的聯繫方式，該APP會利用資料庫將需要服務的居家老人需求記錄起來，並且在APP上設計多種預防照顧預約功能，諸如：電話問安、餐飲服務、健康促進、緊急救助等功能，除此之外，為測試居家老人與社區關懷連結APP實際測試情形，本研究未來將規劃在嘉南藥理大學附近社區場域實際操作，並且實施問卷調查其使用效益與便利性，作為未來改善之依據，本研究冀望可以提供一個友善且便利的居家老人與社區關懷連結APP，以達到國家長期照護在地老化的目標。

This study focuses on the issue of aging population. Our government develops many elder policies in recent years, including Long-Term care project 2.0. This project is to achieve aging in place. This paper study one term, community care, in that project, achieving aging in place. This paper wants to develop a APP to facilitate the implementation of community care. This APP stores the elders' needs in the database and design several care services such as Catering service, phone contacting, activity service, etc. We also implement this APP in a community near CNU and analyze the effect of this APP. We hope this APP can provide a friendly and convenient interface between the elder and community to achieve aging in place.
