摘要

背景：藉由資料分析跌倒原因為病人雙下肢肌力不足、照顧者缺乏正確轉位技巧、病人與照顧者預防跌倒認知不足、照護團隊無建立共同預防跌倒計畫、缺乏適合篩選腫瘤人進行復健評估工具等。目的：降低腫瘤病人跌倒發生率由0.14%降至0.09%以下且無中重度跌倒傷害。解決方案：經由跨團隊合作加強病人及照顧者對預防跌倒認知、進行照顧者協助病人正確轉位技巧、製訂照會復健流程、新增「防跌措施篩選工具」等。結果：結果顯示跌倒發生率由0.14%降至0.07%且無中重度傷害、護理師對防跌措施篩選工具滿意度90.0%、照護團隊對照會流程滿意度92.0%、病人接受復健執行率54.4%。結論：期盼透過專案推動及平行推行至所有腫瘤病房，以維持專案成效，提供病人更安全的就醫環境。

The purpose of the protocol was to apply the cross-team cooperation to reduce the fall rates in cancer patients. Through the data analysis, the factors associated with the patients fall rates were: bilateral lower limbs weakness, inadequate care-giving translocation skill as well the ignorance of preventing falling down, lacks of knowledge on preventing fall events, inadequate interperfessional team care plan for preventing fall, and lacks of tools for screening cancer patients rehabilitation assessment. Through team cooperation, we enhanced the cognition of the patients and caregivers about the preventing of fall, through educating caregiver`s translocation skill to prevent fall, made the process for consulted rehabilitation department, added to the tools for screening intervention on preventing fall. The results shows the fall rates in the patients decreased from 0.14% to 0.07%, nursing staff tools for screening intervention on preventing fall satisfaction 90.0%, interperfessional team process for consulted rehabilitation department satisfaction 92.0%, the patients accepted rehabilitation intervention completion rate 54.4%. We hope that this project can be applied to other oncological wards to ensure patient`s safety.
