提升門診病人居家化療溢灑處理認知正確率

Enhancing Outpatients' Recognition of Handling Chemotherapy Drug Leakage at Home

摘要


背景:化療藥物具致癌性與毒性,若不慎溢灑除會造成環境汙染外,且可能危害病人與他人之健康,故本方案目的旨在提升門診居家化療病人藥物溢灑處理認知正確率。經分析導致病人處理居家化療溢灑認知低之原因為:1.未建立居家化療溢灑處理標準作業規範;2.未將居家化療溢灑納入護理指導項目;3.缺乏居家化療溢灑護理指導之工具;4.未安排護理師居家化療溢灑教育訓練;5.缺乏居家化療溢灑之處理用物。目的:提升病人居家化療溢灑處理認知正確率由改善前60.7%提升至改善後之90.0%。解決方案:實施策略為:制定居家化療溢灑處理標準作業規範及護理指導單張、擬定居家化療溢灑處理流程、設計居家化療溢灑包、將居家化療溢灑處理納入護理指導規範及舉辦護理師教育訓練。結果:病人居家化療溢灑處理認知正確率由專案實施前之60.7%提升至實施後之96.4%。結論:本專案不但提升病人居家化療溢灑處理的認知,且讓病人了解基本自我防護與處理方法,如此不但能減少病人暴露在危險的環境中,且可使病人更安心、更安全地在家接受化學治療。

Chemotherapy drugs are poisonous and carcinogenic. Therefore, once they are leaked or spilled, they would pollute the environment and also may threaten the health of patients and others. As a result, the project aimed to enhance outpatients' accuracy of the recognition on handling chemotherapy drug leakage at home. According to the analysis, the reasons leading to outpatients' low accuracy of recognition on handling chemotherapy drug leakage at home included: (1) the operational standard of dealing with chemotherapy leakage at home was not established, (2) patients' education did not include how to handle chemotherapy drug leakage at home, (3) the instruments of teaching how to handle the leakage were insufficient, (4) no corresponding education and training were provided for nurses, (5) the kits to handle chemotherapy drug leakage at home were not offered. To solve the problem, the strategies provided included: (1) to establish the operational standard of dealing with chemotherapy leakage at home and educational materials for nursing, (2) to set up procedures for dealing with chemotherapy drug leakage at home, (3) to design a package for chemotherapy drug leakage at home, (4) to make home chemotherapy drug leakage as part of patient education, and (5) to offer education and training for nurses. The outpatients' accuracy of recognition on handling chemotherapy drug leakage at home increased from 60.7% to 96.4%. The project not only improved outpatients' recognition on handling chemotherapy drug leakage at home, but also provided efficient ways for outpatients to reduce the risk of exposure to dangerous environment. Furthermore, the project also helps patients to feel more comfortable and safer to receive chemotherapy at home.
