院內發生跌倒事件不僅影響病人安全,增加醫療成本,甚至可能延長病人住院天數。本單位為某醫學中心急診部,跌倒率偏高造成病人產生除原有疾病外之額外傷害,引發專案小組改善動機。經現況分析急診跌倒率高之原因為:病人及照顧者防跌認知不足、護理師不熟悉高危跌倒藥物、未接受防跌教育訓練、防跌衛教單張內容不符現況、輔具取用不便及防跌人力未分工。本專案實施舉辦醫護防跌在職教育課程、高危跌倒藥物建檔及宣導、輔具置放區及設置高危防跌觀察區、全面使用防跌衛教單張與防跌警示掛牌,以及防跌醫護聯防系統之應用等。上述措施介入後,急診病人跌倒發生率由0.058%降至0.027%,有效降低跌倒發生率,不但維護了病人安全也同時提升了整體照護品質。
In-hospital falls impact patient safety, increase medical costs, and can even prolong the length of hospitalization. The increased incidence of falls in the emergency department of our medical center prompted the task force to review and work to improve the current situation. We analyzed several factors associated with a high fall rate in the emergency care unit. First, both patients and caregivers do not understand the necessity of preventing falls in the emergency unit. Second, our unit does not provide appropriate fall prevention training, and the nurses are not familiar with the medications that exacerbate the risk of falls. Third, the existing fall prevention posters are not in line with the current knowledge of risks related to falls. Finally, in addition to inappropriate management of the environment and the limited availability of medical accessories, comprehensive support mechanisms in the healthcare team are lacking. Therefore, the objectives of this project include: conducting an in-service program for fall prevention for the healthcare team, documenting and circulating materials on the medications that increase the risk of falls, reserving a special observation area for patients at high-risk of falls, assigning an area for accessory tools, displaying fall prevention posters along with appropriate caution signs, in addition to developing other support mechanisms in the healthcare system. With such a system in place, we observed a decreased incidence of falls in the emergency unit from 0.058% to 0.027%. Such measures have been successful in enhancing patient safety and improving the quality of care services by effectively reducing the incidence of falls.
