腸道複製囊腫(duplication cysts)是一種罕見之消化道平滑肌病變,患部易發生感染,使得病童常因發燒、腹痛、噁心嘔吐、便秘等腸胃症狀住院治療。本文個案即為患有此症之2歲幼童,於2019年5月23日至6月7日的護理期間發現個案在全靜脈營養支持及抗生素治療下,仍因反覆的腸胃道症狀及腹痛,嚴重影響到進食、活動力、情緒及睡眠,亦讓案父母感到擔憂。於常規治療下,輔以過去修習中醫護理之知識並查閱相關文獻後,結合薰衣草精油芳香療法及中醫小兒按摩技巧與五臟六腑之表裏相對應概念,避開腹痛及發炎部位,按壓能止痛及調節消化道系統之相對應穴位,包括:魚際、合谷、神門、內關、足三里、三陰交等穴位,進行每天二次,每次15鐘之按摩,使得個案的腹痛程度減輕,食慾及睡眠皆獲得改善,同時亦降低案母的無助感及照顧壓力。期望此結合中醫穴位按摩及芳香療法於緩解病童腹痛及腸胃道症狀的照護過程,能提供臨床護理師照護腸胃道疾病兒童之參考。
Enteric tract duplication cysts are a rare congenital gastrointestinal malformation. The typical gastrointestinal symptoms and signs include recurrent infections, abdominal pain, nausea and vomiting, and constipation. These symptoms may require patient hospitalization. The patient in this report was a 2-year old girl experiencing abdominal pain and gastrointestinal symptoms caused by duplication cysts, even under medical treatment with total parenteral nutritional supplementation and antibiotics. The nursing period was from May 23 to June 7, 2019. Recurring gastrointestinal symptoms and abdominal pain not only seriously affected the patient's food intake, activity, mood, and sleep, but also caused her parents much worry. In addition to the medical treatment, we attempted to resolve her abdominal pain and symptoms by using traditional Chinese medicine, considering hers and her parents' needs and preferences and our nursing experience with traditional Chinese medicine. We referred to the literature on traditional Chinese medicine and introduced lavender oil aromatherapy massage along with Chinese acupressure, which were based on the concept of "internal and external related channels." This implies compression on the acupoint instead of the abdominal pain and inflammation area to relieve the abdominal pain and gastrointestinal symptoms of the patient. Acupuncture points associated with analgesic and digestive system regulation, include Yuji, Hegu, Zusanli, Sanyinjiao, Shenmen, and Neiguan. The frequency of the massage was twice a day, and each session lasted 15 minutes. After the intervention, the patient's abdominal pain decreased, her appetite and sleep quality improved, and gastrointestinal symptoms were resolved. Furthermore, the helplessness and care pressure of parents were alleviated. This nursing experience offers a reference for nurses on using alternative methods to manage gastrointestinal disorders in children.
