運用行為修正模式降低手術恢復室環境噪音之專案

Reducing Ambient Noise using Behavior Modification in the Postoperative Intensive Care Unit

摘要


背景:環境噪音容易使人心跳加速、血壓升高及躁動,不僅降低護理工作效率,更影響病人術後休息。2020年6月手術病人對環境舒適滿意度為80%,其中噪音滿意度僅18%,且時常有病人抱怨。分析噪音源為護理人員交談聲及隔空喊話、清潔人員清床聲及開關汙衣桶、病童哭聲及鄰近病人聲音、開關櫃子抽屜、儀器聲響及電話門鈴聲。目的:降低環境噪音至70分貝以下及提升環境噪音滿意度至80%。解決方案:運用行為修正模式包含警示海報標語、舉牌、制定作業規範、舉辦在職教育及加裝緩衝防震設備。結果:噪音從平均77.3下降至57.4分貝,環境噪音滿意度由18%提升至84%。結論:運用行為修正模式可有效改善手術恢復室環境噪音,建議推廣運用在鄰近兒童外科加護單位,除可提供病人安靜休息的環境,更創造醫院更優質的工作環境。

Background: Ambient noise can easily stimulate a rapid heartbeat, increase blood pressure, and cause irritation. Furthermore, it is inefficient for nursing and affects the patient's recovery. In June 2020, the patient had several complaints. While the patient's satisfaction with environmental comfort was 80%, satisfaction with ambient sound received a mere rating of 18%. The analysis of ambient noises was informed by the communication among nursing staff, loud shoptalk, as well as sounds from cleaning the mattress, opening the laundry basket, crying children, patient voices, opening and closing the drawers and cabinets, instruments, and doors and ringing phones. Purpose: To reduce noise levels to below 70 dB and improve satisfaction with ambient sounds to exceed 80%. Methods: To implement job training, anti-vibration equipment, holding and placing posters with warnings or slogans, and establishing codes of practice among other behavior modifications. Results: The average noise decreased from 77.3 dB to 57.4 dB. Furthermore, satisfaction with ambient sound increased from 18% to 88%. Conclusions: Behavioral modifications can improve ambient noise in the postoperative intensive care unit. Thus, a similar method is recommended to promote a quiet atmosphere for patients and improve the working environment for health professionals in other departments.
