運用治療性遊戲於一位學齡期兒童隱睪症術後之恢復室照護經驗

Recovery Room Nursing Experience Involving Therapeutic Play for a School-age Child with Cryptorchidism after Orchiopexy Surgery

摘要


本文為照護一位七歲男童隱睪症術後於恢復室之經驗。照護期間為2020/2/19 11:45 am至12:55 pm,以直接照護、觀察、訪談及病歷查閱等方式收集資料,確認個案有疼痛、焦慮,及案母有照顧者角色緊張之健康問題。照護期間,提供止痛藥物及非藥物處置,使用溫柔語調、提供個案喜愛的影片及案母陪伴,成功轉移個案對疼痛之注意力亦使疼痛得到緩解;運用治療性遊戲,配合個案最愛的卡通玩偶進行角色扮演,使瞭解各項處置用意,焦慮逐漸得到緩解,也可配合治療;針對照顧者角色緊張,給予同理、傾聽及支持,讓案母提出心中疑問及擔憂,予以解答並適時澄清疑慮,增加案母照護自信並減少緊張感。經由照護經驗,建議護理師應加強治療性遊戲之專業知識、於衛教單張中增加圖片並製作衛教影片,提升病人或家屬對術後照護的瞭解。

This article describes the recovery room nursing experience involving caring for a school-age child with cryptorchidism using therapeutic play after orchiopexy surgery. The care period occurred from 11:45 to 12:55 on February 19, 2020. The data were collected based on direct care, observation, interviews, and reviewing medical records. Consequently, the data confirmed that the patient suffered from pain and anxiety while his mother suffered from caregiver role strain. During the care period, the medical staff administered painkillers and performed non-drug treatment, which includes using a soft tone for communication. Furthermore, the patient was allowed to watch animated cartoons of his preference and had access to his mother's company, which successfully diverted his attention and alleviated the wound pain. Therapeutic play, including role-playing using the patient's favorite cartoon dolls, was performed to help the patient understand the purpose of treatment and gradually eliminate anxiety. Ultimately, the patient's willingness to cooperate with the treatment increased. To remove the caregiver role strain, the nurses provided empathy, attentive listening, and support in addressing the mother's questions and concerns by prompting answers to the mother to eradicate doubts, enhance confidence in caring, and reduce tension. Thus, this study recommends strengthening the professional knowledge of nurses regarding therapeutic play. Furthermore, it is pertinent to increase illustrations in nursing education to deepen the understanding among patients and caregivers of postoperative care.
