降低因疫情需求病房轉型護理師之跨科照護壓力

Reducing the Work-Related Stress on Nurses Required to Perform Interdisciplinary Nursing Care in Response to the COVID-19 Pandemic

摘要


背景:因應2021年5月COVID-19於社區突發之疫情,使本病房於疫情高峰期間需轉型為照顧不同科別之內科疾病後送病房,造成護理師壓力值高,壓力源量表平均達68.0分,因此專案旨在降低護理師之跨科照護壓力。專案執行期間為2021年5月21日至8月30日。分析造成壓力高之原因為:擔心無法掌握病人病況、內科疾病/藥物生疏、護理技術/常規不熟悉、上班時數增加、人力不足無法休假、無可諮詢的對象等等。目的:希望藉此專案改善措施,能將護理師於跨科照護其間之工作壓力源量表由68.0分降至54.4分。解決方案:為了讓護理師短時間內調整情緒及工作壓力故成立專案小組,藉由舉辦課程、CbD討論、技術評核、短期交叉訓練、調降護病比、安排成組護理、整理防疫手冊等。結果:專案實施後,共16位護理師於跨科照護期間其壓力源量表由平均平均68.0分降至50.6分,同時也提升護理師對於不同科別照顧能力。結論:希望藉此專案之改善可作為未來因應傳染病疫情變化需調整病房照護型態之參考。

In May 2021, a COVID-19 outbreak occurred in Taiwan. In response to the sudden increase in cases, our hospital selected our ward to be immediately transformed into a unit caring for patients with various conditions. Unfamiliarity with interdisciplinary patient care and collaboration among diverse medical teams can lead to stress on nurses; the stress score of nurses in the ward was 68.0. The study period was May 21 to August 30, 2021. The top stressors among the nurses were the fear of being unable to comprehend a patient's condition, unfamiliarity with diseases and drugs, unfamiliarity with nursing techniques and routines, increased working hours, insufficient nursing staff, the inability to take time off, and the lack of authorities to consult when confronted with difficulties. To reduce the work-release stress on nurses to perform interdisciplinary care, and the average stress score decreased from 68.0 to 54.4. This study first established a working team, evaluated stressful conditions and stressors, designed improvement measures, selected feasible solutions, and finally implemented interventions. Seven strategies were proposed for reducing the impact of the aforementioned stressors: (1) implementing educational training, (2) hosting CbD discussions, (3) re-evaluating nurses' skills, (4) implementing short-term cross-training, (5) reducing the patient-to-nurse ratio, (6) arranging team care, and (7) s preparing prevention manuals. A total of 16 nurses were included, and their stress scores were collected. A posttest revealed that the nurses' average stress score had decreased from 68.0 to 50.6 and that their interdisciplinary care abilities had improved. The results of this study can be used as a reference for developing future nursing policies in response to various challenges.
