Alcoholic Ketoacidosis with Severe Hyperglycemia Mimicking Diabetic Ketoacidosis- A Case Report

In case of patients with ketoacidosis, it is important to differentiate between diabetic ketoacidosis (DKA) and alcoholic ketoacidosis (AKA), particularly in alcoholics. Patients with AKA typically have normal or low blood glucose levels. However, we observed an AKA patient with severe hyperglycemia (32.4 mmol/L) mimicking DKA. The 57-year-old alcoholic male presented with an episode of ketoacidosis after recent binge drinking. Insulin treatment resulted in the rapid correction of hyperglycemia and ketoacidosis. He had a normal blood hemoglobin A_(1c) (HbA_(1c)) level, and high fasting plasma Cpeptide level (1.41 nmol/L) immediately after the episode and near normoglycemia 1 week after hospitalization. He continued to have a normal blood HbA1c level 8 months after the episode. Severe hyperglycemia detected during ketoacidosis episode does not preclude the AKA diagnosis. Hyperglycemia, irrespective of the degree, cannot be applied to differentiate DKA from AKA.
