探討住院高齡者與其主要照顧者對使用留置尿管之感受

Exploring the Perceptions about Use of Urinary Catheters among Hospitalized Elderly Patients and Their Primary Caregivers

摘要


目的:高齡者與主要照顧者對留置尿管的感受會隨使用情境不同而異,且可能影響使用情形,進而增加不當使用。因此,深入了解住院高齡者與主要照顧者對留置尿管使用的感受顯得重要。本研究目的為探討住院高齡者與主要照顧者對留置尿管使用的感受。方法:採個案研究法,立意取樣十位入院後使用留置尿管之住院高齡者與主要照顧者,使其敘述使用尿管至移除後排尿功能恢復之照護歷程與經驗,本文針對其描述之使用尿管原因與感受進行探討,訪談內容以內容分析法整理歸類。結果:住院高齡者與主要照顧者對留置尿管使用的感受內含四個主題:(一)肯定尿管的功能:提升舒適感、照護方便、(二)尿管的負向影響:活動受限制、擔心尿管併發症與減少社會互動的影響、(三)對尿管感到不習慣或不舒服、(四)質疑尿管需要性。結論:本研究結果提供住院高齡者與主要照顧者於使用留置尿管期間的照護需求與感受,有助於醫護人員提供適切照護,協助高齡者及其照顧者因應留置尿管帶來的影響,作為未來發展住院高齡者留置尿管照護方案之參考。

Objectives: While varying from person to person depending on the contexts in which indwelling urinary catheters (IUCs) are used, the perception of hospitalized elderly patients and their caregivers about IUCs may increase inappropriate use. Therefore, it is important to obtain an in-depth understanding of the perceptions about the use of IUCs among hospitalized elderly patients and their primary caregivers. This study aimed at exploring the perceptions of use of IUCs among hospitalized elderly patients and their primary caregivers in Taiwan. Methods: The case study method was used to examine the experiences of using IUCs from insertion to withdrawal among ten recruited hospitalized elderly patients and their caregivers. A semi-structured interview was conducted to help understand the participants' perceptions about IUC use with qualitative data coded and analyzed using content analysis. Results: Four themes emerged from the responses of the participants concerning their perceptions about the use of IUCs: recognizing the benefits of IUCs, worrying about the adverse impacts of IUCs, feeling unaccustomed to or uncomfortable with IUCs, and questioning the need to use IUCs. Conclusion: By providing insights into the perceptions about the use of IUCs among hospitalized elderly patients and their caregivers and their care needs, the results of the study can be expected to help hospitalized elderly patients and their caregivers better cope with the impacts of using IUCs and assist medical and nursing professionals in developing effective interventions and providing appropriate care for using IUCs in elderly patients.
