周全性老年評估個案報告：老年喪偶以後心房顫動及急性心衰竭為表現的

本案例介紹一68歲男性，過去健康狀態良好，慢性病亦規則控制。在經歷喪偶後，兩個月內陸續功能減退，體能下降，出現心房顫動及急性心衰竭而住院治療，經由老人周全性評估，除了就心臟問題予以檢查治療外，仔細查明其背後傷慟可能的複雜性悲傷，憂鬱情緒等促發因子。以包含整體身心壓力，生活形態的評估來重新省視此一心臟疾病事件。病患最終在藥物治療，以及約一年的適應後回復良好。藉由本個案，點出在逐年上升喪偶比例的台灣現代社會，需要積極識別這一段特殊的老年喪偶期，在喪偶的過程中給予安寧緩和醫療介入，善生善別；喪偶後有積極適當的家庭社會支持，遺族輔導。必要時轉介心理諮商，或藥物協助，來避免可能引發的後續健康問題。

This case introduces a 68-year-old man who had chronic diseases under controlled and in good health status. After the death of his wife, within two months, he had developed exercise intolerance and functional decline. He was admitted to a geriatric ward for evaluation. Atrial fibrillation and acute heart were diagnosed and given proper treatment. A Comprehensive Geriatric Assessment (CGA) was done, which revealed that in addition to the heart problems, the complexity of grief, depression and bereavement also seem to trigger the 'broken heart' event. The patient eventually recovered well after treatment and about one year of adaptation. Through this case, we point out that in the modern society of Taiwan, we need to actively identify this special period of widowhood in the elderly. By given timely psychological counseling or medical assistance, we may avoid possible subsequent health problems.
