與高齡者進行預立醫療照護計畫討論之技巧

Skills for Discussing Advance Care Planning with Older Adults

摘要


多重共病與衰弱失能的高齡長者隨人口老化快速增加,因病程常持續多年且變化多端,難以預測其預後,因此進行預立醫療照護計畫(ACP)的討論充滿了挑戰。ACP討論能幫助個案獲得想要的醫療照護方式,以提升生命末期的生活品質、減少個案與家屬的焦慮或憂鬱、且可降低生命末期醫療照護成本。若過慢或無討論將無法獲得上述的優點,因此即早開啟與高齡者的ACP對話有其重要性。臨床人員可透過學習溝通ACP的技巧,即可克服影響討論ACP之阻礙,提升與個案進行溝通的意願。本文整理出可簡單依循的六個步驟,希望藉由結構性的指引,幫助臨床專業人員在照護過程中,有信心主動邀請高齡者與其照顧者參與ACP討論,以便能預留時間進行思考與決策的制定,後續促使相關預立醫療決定的簽署,以獲得符合其心願的治療與照護方式。

As our population aging, older people live longer with multi-morbidity and frailty. Prognostication in this group remains difficult, making advance care planning (ACP) discussion challenging. ACP discussion increases compliance with patients' end-of-life wishes, quality of care, and reduce anxiety and depression of patients and family. It is important to start the ACP discussion early to prevent negative outcomes associated with delayed communication. Clinicians can overcome barriers of ACP discussion by adopting practical and systemic steps to engage older adults in the ACP process. In this article, we introduce a six-step approach to ACP discussion. We hope that the suggestions provided here will enable greater ACP discussion and documentation of advance directives in clinical settings for older adults.
