戒癮心理門診計畫：以科學實證來發展矯正機關的戒癮處遇模式

摘要

在矯正機關面對非自願性案主工作時，經常會囿於當事人改變意願低落、抗拒甚至是敵意問題而使得處遇效果不佳，然而美國藥物濫用研究所根據數十年實證研究經驗彙整所出版的藥癮治療原則，其中一項便已清楚揭示：「非自願性的戒癮治療亦能呈現療效」；因此，在專業人力有限、藥癮治療需求龐大的矯正系統中，如何提供適切的戒癮服務，便成為當前挑戰。本文係分享高雄戒治所近十年來發展「戒癮心理門診」模式的經驗，其內容亦根據美國藥物濫用研究所出版的藥癮治療原則為基礎來設計，同時以實證研究來進行療效評估，並根據上述原則的修編及研究結果，前後共進行三次戒癮心理門診流程與處遇計畫的修訂，期可持續改善療效；希望在自曝其短、拋磚引玉的文字說明後，能持續與各位先進共同致力提升矯正機關戒癮處遇的效益，讓藥癮者賦歸社會後得以擁有更多的無毒生活契機。

An issue of providing appropriate treatment services has become a challenge to many correctional institutes due to their limited professional workforce, who always strives to respond to overwhelming treatment workload. The cause of this issue is related to poor treatment efficacy, since those involuntary drug addicts, with poor motivation and strong hostility, are often reluctant to change their behavior. However, ＂Principles of Drug Addiction Treatment,＂ published by the U. S. National Institute on Drug Abuse (NIDA), states that ＂Treatment does not need to be effective.＂ which serves as a solid foundation for this study. The present paper provides the experience of our abstinence treatment model implemented by Kaohsiung Drug Abuse Treatment Center over the past decade. Based on the principles of NIDA, a research-based experimental study, which undergoes three revisions for the clinical drug abuse treatments, serves to evaluate and improve treatment efficacy. It is hoped that our treatment projects will receive constant review and revision in order to enhance curative effect. In the long run, our goal is to help addicted individuals achieve long-term abstinence and stay at a drug-free life.
