影響醫療人員持續採用電子病歷之實證研究:結合後接受模式與科技準備和接受模式

Empirical Study of Factors Influencing Medical Personnel's Continuous Use Intention of Electronic Medical Records: Combining the Post-Acceptance Model and Technology Readiness and Acceptance Model

摘要


目的:科技化時代的來臨,世界各地的醫院都在使用電子病歷來改善病患安全、提高照護品質和效率。然而,關於台灣醫院醫療人員對電子病歷持續使用意圖的影響因素知之甚少。本研究目的旨在探討影響醫療人員對電子病歷持續使用意圖之因素,並以後接受模式為基礎,結合科技準備和接受模式。此外,這項研究還提出科技準備度是一種具有高階結構的多維構造之概念。方法:研究數據來自中部地區兩家醫院之204位醫療人員,並使用結構方程模型進行分析。結果:分析結果發現:(1)知覺有用性、滿意度對持續使用意圖呈現顯著正向影響;(2)知覺有用性、確認對滿意度呈現顯著正向影響;(3)科技準備度指定為形成結構,並直接影響知覺有用性、確認、知覺易用性、持續使用意圖,這種現象以前從未被探索過;(4)樂觀性、創新性、不適應性、不安全性為影響科技準備度的重要因素。結論:最後,本研究針對理論模式與後續研究提出建議。

Objective: Hospitals worldwide use electronic medical records (EMRs) to improve patients' safety and the quality and efficiency of care. However, few studies have investigated the factors that influence medical personnel's intention of continuing their use of EMRs. Therefore, this study investigated such factors by using a combination of the post-acceptance model and the technology readiness and acceptance model. This study proposed that technology readiness is crucial for determining medical workers' adoption and use intention of new technology. Methods: Data were collected from 204 medical workers in two hospitals in central Taiwan and analyzed through structural equation modeling. Results: The following results were obtained: (1) Perceived usefulness and satisfaction had significantly positive effects on the continuous use intention of EMRs; (2) perceived usefulness and confirmation had significantly positive effects on satisfaction; (3) technology readiness was a formative construct that directly affected confirmation, perceived ease of use, continuous use intention, and perceived usefulness; and (4) optimism, innovativeness, discomfort, and insecurity were key factors affecting technology readiness. Conclusion: This study discussed limitations and implications for further research and practice.
