Long-term Prognosis in Spontaneous Intracerebral Hemorrhage Survivors

Intracranial hemorrhage consists of a quarter of stroke, and manifests with severe consequence, high morbidity and mortality. Sporadic small vessel disease is the predominant underlying etiology, and the most common subtypes are hypertensive small vessel disease and cerebral amyloid angiopathy. The clinical and radiological markers predicting early prognosis have been well identified, but the long-term prognosis including long-term mortality, cognitive performance, ambulatory and function decline is less studied in intracranial hemorrhage survivors. Factors related to the long-term prognosis may be also resulted from the combined effects of damage during the acute phase of intracerebral hemorrhage, pre-existing parenchymal damage from underlying microangiopathy, and their other comorbidities. In this article, we will briefly introduce the types of small vessel disease underlying spontaneous intracerebral hemorrhage, and how they affect the long-term intracerebral hemorrhage recurrence and morality. We will also discuss the presentation of cognitive impairment and the neuropsychiatric manifestations in intracerebral hemorrhage survivors.
