發展遲緩兒童之中醫治療臨床案例報告

摘要

本案例是一位2歲女童，在花蓮慈濟醫院兒童發展復健中心評估顯示有多項發展遲緩後，隨即被安排進行復健治療。在進行復建期間，於2017年8月至中醫門診尋求幫助。女童於第一次中醫門診時表現出無法站立、無法扶牆步行等粗細動作的遲緩，並同時伴隨體重過輕及睡眠時間很短等情況。經中醫診察後，以中藥、頭皮針、雷射針灸及捏脊進行治療。中醫持續治療2個月期間，睡眠狀況時間逐漸拉長。患童持續進行西醫復健合併中醫治療後的3個月，在粗、細動作上皆有進步；合併治療後的8個月，可在無攙扶狀況下步行。本案例詳細記錄施予發展遲緩患童的各項中醫療法，供同道日後做為治療同類疾病的參考，並期許中醫為更多發展遲緩兒童提供幫助。

The case report focused on a 2-year-old female patient failing to walk with significant developmental delays. According to the evaluations from Child Development and Rehabilitation Center of Hualien Tzu Chi Hospital, the patient was arranged to do some rehabilitation. During rehabilitation, the patient accepted the treatment in the outpatient of traditional Chinese medicine (TCM) in August, 2017. At her first admission, the patient was manifested as failure to stand alone or walk with supports, simultaneously accompanied by underweight and insomnia. Based on four diagnostic methods as well as pattern differentiation of TCM, a series of therapies were applied, including TCM formula, scalp acupuncture, laser acupuncture, and infantile tuina. During 2 months of TCM treatment, the patient's insomnia was improved. Three months later of integrated treatment of TCM and Western medicine, the patient got some progress in both fine and gross motor function. The patient walked without supports in the ninth month after her first visit. The integrated treatment of TCM and Western medicine, in this case, can not only provide a reference for the further treatments of the same diseases but also offer assistance concerning TCM to more children suffering from developmental delay.
