中藥合併針刺與雷射針灸治療小兒遺尿案例

小兒遺尿是兒童常見生活問題之一，為5歲以上兒童出現不能自主控制排尿，常於睡夢中小便自遺，醒後方覺的一種疾病。現代醫學針對1週尿床2天以上的孩童，在排除器質性病變後，建議有非藥物及藥物治療。本報告呈現一位8歲男童尿床案例，因西藥治療效果不佳，希望能以中醫治療以達更佳療效，故而至中醫門診求診。經診斷為腎氣不足兼心火亢盛，以中藥合併針刺及雷射針灸治療。於治療4週後，尿床症狀明顯改善。惟病人在療程中未完全改善即因故無回診，而是在6個月後因症狀再次加重而回診，且治療時出現服藥順從性較低之狀況。在臨床的觀察上可以發現，後段治療的改善時程似乎較為拉長，前後兩期的治療差異可做為療程規劃上之經驗與參考。

Nocturnal enuresis, otherwise known as bedwetting, commonly afflicts children aged 5 and older while they sleep at night. Various medications or other alternatives are prescribed to children not showing any organic problems. In this case report, we describe outcomes for an 8-year-old boy suffering from nocturnal enuresis who was diagnosed with Qi deficiency of the kidneys and heart fire hyperactivity. After 4 weeks of combinatorial treatment with Chinese medicine herbs, acupuncture, and laser acupuncture, a significant decrease in recurring symptoms was observed. However, compliance with the treatment protocol was disrupted before all symptoms had disappeared, causing the patient to relapse after 6 months. Therefore, the efficacy of later treatments was reduced and their duration needed to be prolonged in order to achieve the same effect as earlier. This case report demonstrates the potential of a combinatorial therapy to adequately treat nocturnal enuresis, and the findings can serve as a reference for future remedies or treatment plans.
