萬古黴素用藥情況調查及分析

目的：分析鏡湖醫院萬古黴素臨床應用情況，為進一步促進萬古黴素臨床合理應用提供參考。方法：採用回顧性研究方法，調查鏡湖醫院2020年1月～12月使用萬古黴素的住院患者病歷資料，對患者基本資料、病原學檢查，血藥濃度監測及不良反應等情況進行匯總及分析。結果：50例患者中，老年患者佔68%，萬古徽較多應用於呼吸系統感染（56%），78%患者聯合應用抗菌藥物，血藥濃度監測率為78.72%，在治療窗的比例佔54.64%，3例病人發生急性腎損傷。結論：本院萬古黴素血藥濃度監測率有待提升，臨床宜結合患者具體情況制定給藥方案並及時參考血藥濃度結果進行調整，警惕藥物不良反應。

Objective: To survey the clinical application of Vancomycin in Kiang Wu Hospital, thus providing a reference for reasonable administration of Vancomycin clinically. Methods: A total of 50 patients who received Vancomycin during hospitalization in Kiang Wu Hospital between January and December 2020 were included in a retrospective study of their medical records. The study summarized and analyzed the patients' general information, etiological findings, therapeutic drug monitoring results, and adverse reactions. Results: Among the patients, 68% were elderly cases, for whom the drug was mostly used for respiratory infection (56%), and for 78% of the patients, the drug was combined with other antibiotics. The therapeutic drug monitoring rate was 78.72%, 54.64% within the therapeutic window. Three patients developed acute kidney injury. Conclusion: The monitoring rate in elderly patients improved in Kiang Wu Hospital. The dosing regimen should be adjusted concerning the blood concentration results and beware of adverse drug reactions.
