LASIK術後白內障人工晶體度數測量公式比較

Comparison of intraocular lens power calculation formulas in cataract eyes after LASIK

摘要


目的:分析評估LASIK術後白內障患者應用Shammas-PL、Haigis-L、Barrett True K三種公式預測人工晶體度數的準確性。方法:收集既往曾經做過LASIK激光矯視手術,2019年7月~12月在本院行白內障超聲乳化及人工晶體植入術的連續病例共7眼,使用Shammas-PL、Haigis-L、Barrett True K公式測算人工晶體度數。術後隨訪3個月,觀察術後視力、矯正視力和屈光狀態。對三種人工晶體測算公式進行比較,觀察其應用效果。結果:Shammas-PL、Haigis-L、Barrett True K公式的預測屈光度與術後實際屈光度誤差平均值別為0.07D、0.33D和0.17D,三種公式的預測屈光誤差平均值均>0,說明應用這三種公式測算的人工晶體度數,在白內障術後屈光度屬於欠矯。預測屈光誤差絕對值分別為0.98D、1.19D和0.58D,Barrett True K的預測屈光誤差絕對值最小。而預測屈光誤差絕對值位於1.00D之內的比例分別為57.1%、71.4%和100.0%。三種公式預測屈光度與術後實際屈光度比較差異無統計學義意,而三種公式預測屈光誤差之間的差異同樣無統計學意義(P >0.05)。結論:Shammas-PL、Haigis-L及 Barrett True K公式預測LASIK術後白內障人工晶體度數的準確度一致性好、是安全、可信的,Barrett True K公式的準確性最好,三種公式均有術後屈光度欠矯情況。

Objective: To analyze and evaluate the accuracy of using Shammas-PL, Haigis-L and Barrett True K to predict the index of intraocular lens in cataract patients after LASIK surgery. Methods: A total of 7 consecutive cases of cataract phacoemulsification and intraocular lens implantation performed in our hospital from July to December 2019 were being collected, using the Shammas-PL, Haigis-L, and Barrett True K formulas to measure the index of the intraocular lens. The postoperative follow-up was three months to observe the postoperative vision, corrected vision and refractive status. The three calculation formulas of three intraocular lenses are compared by their application effects. Results: The average values of the predicted refractive power and the actual postoperative refractive power errors of the Shammas-PL, Haigis-L, and Barrett True K formulas are 0.07D, 0.33D, and 0.17D, respectively. The average predicted refractive errors of the three formulas are all> 0, indicating that the power of the intraocular lens measured is under-corrected after cataract surgery. The absolute values of predicted refractive errors are 0.98D, 1.19D, and 0.58D, respectively. Barrett True K has the lowest absolute value of predicted refractive errors, and the proportions of predicted refractive errors within 1.00D are 57.1%, 71.4%, and 100.0%. The difference between the predicted refractive power of the three formulas and the actual refractive power after surgery is not statistically significant, and the difference between the predicted refractive errors of the three formulas is also not statistically significant (P >0.05). Conclusion: The formula of Shammas-PL, Haigis-L and Barrett True K have good consistency in predicting the index of cataract intraocular lens after LASIK, and they are safe and reliable. The accuracy of the Barrett True K formula is the best, and among the three of them, post-diopter under correction happens after surgery.
