GPC3蛋白在肝細胞癌的表達及與臨床病理特徵的關係

Expression of GPC3 protein in hepatocellular carcinoma and its relationship with clinicopathological features

摘要


目的:檢測肝細胞癌(HCC)、癌旁肝組織和肝血管瘤旁正常肝組織中癌胚蛋白GPC3的表達及分析其與臨床病理特徵之間的關係。方法:收集肝臟手術標本共414例,包括HCC 315例(內含HCC癌旁肝組織183例)和血管瘤旁正常肝組織99例。製作組織芯片及採用免疫組織化學法檢測GPC3蛋白在HCC、癌旁肝組織和正常肝組織的表達情況,結合臨床病理資料進行統計分析。結果:GPC3在315例HCC中呈現不同程度陽性,陽性表達率為70.8%(223/315)。183例HCC癌旁肝組織和99例血管瘤旁肝組織均呈陰性反應。GPC3在HCC中的陽性率明顯高於癌旁肝組織及血管瘤旁肝組織,且與血管侵犯、腫瘤分化程度及血清AFP水平、HBsAg感染有明顯相關性(P<0.05)。結論:GPC3在HCC中表達的高靈敏度和高特異性,是診斷HCC的一個良好標記物。GPC3在HCC中的表達與血管侵犯、腫瘤分化程度有明顯相關性,提示GPC3高表達與HCC更差的預後有關。由於GPC3在HCC中特異性表達,因而可成為免疫治療的靶點。

Objective: To investigate the expression of GPC3 protein in hepatocellular carcinoma (HCC), paracancerous liver tissue and normal liver tissue adjacent to hemangiomas, and to analyze its correlation with clinicopathological features. Methods: A total of 414 clinical specimens were collected to perform tissue microarray containing 315 cases of HCC (including 183 cases of paracancerous liver tissue) and 99 cases of normal liver tissue adjacent to hemangiomas. The expression of GPC3 was detected by immunohistochemistry. The relationship between GPC3 and clinopathological features was analyzed. Results: GPC3 showed various degrees of response in 315 cases of HCC, and the positive rate was 70.8% (223/315). GPC3 was negative in 183 cases of paracancerous liver tissue and 99 cases of normal liver tissue adjacent to hemangiomas. The positive rate of GPC3 in HCC was significantly higher than that in paracancerous liver tissue and normal liver tissue adjacent to hemangiomas. GPC3 expression was significantly correlated with vascular invasion, tumor differentiation, serum AFP level and HBsAg (P<0.05). Conclusion: GPC3 has high sensitivity and specificity in HCC and is a good marker for diagnosis. The expression of GPC3 in HCC was significantly correlated with vascular invasion and tumor differentiation, suggesting that GPC3 overexpression is associated with a worse prognosis. The specific expression pattern of GPC3 in HCC may become a target for immunotherapy.
