186例維持性血液透析患者的臨床特徵及死亡分析
Mortality analysis and clinical characteristic of 186 maintenance hemodialysis cases
摘要
目的：探討維持性血液透析（maintenance hemodialysis, MHD）186名患者死亡的臨床特點、實驗室檢查及死亡原因等資料，並對MHD患者死亡的臨床特徵及相關資料進行分析。方法：收集和整理本院血液透析中心智能數據庫中2016年6月～2020年12月MHD患者的相關資料，並進行回顧性分析。結果：2016年6月～2020年12月本中心MHD患者的年死亡率為5.59%－8.86%。開始透析後的首90天內死亡患者6例（3.22%），全部均為腫瘤患者。死亡患者原發病構成中糖尿病佔51.6%，其次高血壓病（14.0%），慢性腎小球腎炎（8.6%），不明原因（7.5%），其它原因（18.3%）。最常見的死亡原因依次是感染（26.3%），腫瘤（18.8%），心血管事件（15.6%）等。MHD死亡患者的血紅蛋白、血磷及血漿白蛋白與2020年12月仍在生患者相比偏低，並具有統計學意義（P<0.05）。患者Kt/V均達到指南治療標準。結論：2016年6月～2020年12月本中心MHD患者的年死亡率為5.59%～8.86%。糖尿病是MHD患者死亡最常見的原發疾病；感染、腫瘤及心血管事件為最常見的死亡原因。

Objective: To investigate the characteristic of 186 maintenance hemodialysis mortality cases by analyzing their clinical data from June of 2016 to December of 2020. Methods: To collect and analyze the clinical data of all maintenance hemodialysis patients in our center through our Hemodialysis Intelligent Data System from June 2016 to December 2020. Results: The annual mortality rate of MHD patient range from 5.59%-8.86%. The early mortality rate (within 90 days after initiation of hemodialysis) accounts for only six over 186 cases (3.22%), all of which have malignancy. The most common primary disease of a deceased patient is diabetes mellitus (51.6%), followed by hypertension (14%), chronic glomerulonephritis (8.6%), unknown etiology (7.5%), and other causes (18.3%). The most common leading causes of death were infection (26.3%), malignancy (18.8%), and cardiovascular events (15.6%), etc. The hemoglobin level, serum phosphorus level and serum albumin are lower in the deceased patient compared with the MHD patient, with statistically significant. (P<0.05). Most of the patient's Kt/v reached the target of a clinical practice guideline. Conclusion: The annual mortality rate ranged from 5.59%-8.86% from June 2016 to December 2020. Diabetes mellitus is the leading primary disease in deceased patients. Infection, malignancy and cardiovascular event is the most common cause of death in MHD patient.
