降低首次腸造口手術後之照顧者造口袋黏貼滲漏率
Reducing the Stoma Bag Adhesion Leakage for Caregivers Taking Care of Patients Undergoing the Ostomy Surgery for the First Time
摘要
單位照顧者執行造口袋黏貼滲漏率為6.0%，當病人造口袋頻繁滲漏，除增加照顧者壓力外亦造成經濟負擔，更讓病人陷入退縮及社交隔離，引發照護團隊改善動機。經由收集資料分析滲漏原因包括：照顧者缺乏造口護理相關知識、黏貼技巧不熟悉、缺乏衛教相關資訊，於2019年5月15日至2019年11月30日成立專案小組，運用跨團隊資源合作，降低照顧者執行造口袋黏貼滲漏率≦4.5%為目的。運用造口清潔查檢表、尺寸模型卡、護理技巧影片、黏貼衛教單張、剪裁流程簡易口訣等改善措施，將照顧者執行造口袋黏貼滲漏率降低至3.7%，專案推行後降低病人造口袋滲漏，也提升病人生活品質，更將衛教資訊提供全院病房下載，強化跨團隊合作精神。

The leakage rate of stoma bag adhesion on the patients in our unit was 6.0%. In addition to increasing caregivers' stress, the frequent leakage of stoma bags on patients also has caused financial burden, and prevented patients from engaging in social activities, which may result in social withdrawal and isolation. Therefore, the care team's motivation was to reduce the leakage rate. According to the analysis on the collected data, the causes of leakage included the caregivers' lack of stoma care-related knowledge, unfamiliarity with adhesion skills, and the lack of health education-related information. The research group was formed from May 15 to November 30, 2019, and collaborated cross-team resources with the aim to reduce the leakage rate of stoma bag adhesion to ≦4.5%. The improvement measures adopted by the research group included a stoma cleanliness checklist, size-based model card, nursing skill video, posting of health education leaflet, and mnemonic rhyme of the cutting procedure. The leakage rate of stoma bag adhesion performed by caregivers was reduced to 3.7%. The results show that the implementation of those measures successfully reduced the stoma bag leakage rate in patients and improved their quality of life. Moreover, relevant health education information has been provided online for all wards in the hospital to download in order to facilitate cross-team collaboration.
