摘要

2018年開始，台灣老年人口高達14%，正式邁入高齡化社會，國家發展委員會預估，在2026年，台灣會進入超高齡化社會。隨著國民人口結構組成的轉變，安寧醫療的需求逐漸上升。高雄市衛生福利部旗山醫院地處高雄的北端，幅員廣闊，旗美9區65歲以上老年人口約占20％，高齡比率是全台比例最高的地方之一，但醫療資源卻極為缺乏。末期病人經常需要開車到市區求診，無法達成在家往生，在鄉終老的心願。筆者與旗山醫療團隊，於2015年開始，在旗山醫院成立當地第一家安寧照護團隊，提供安寧居家療護和安寧住院服務。以SWOT分析安寧居家療護在旗山醫院實行的情況，並制定策略發展優勢和克服困難。

Taiwan officially becomes an aged society with people over 65 years old breaking the 14% mark. It will become a super-aged society where at least 20 percent of the population is 65 years old or older by 2026. The remote areas of northern Kaohsiung, including Cishan、Meinung、Tianliao、Liouguei、Jiasian、Shanlin、Neimen、Maolin、Taoyuan and Namasia, have become super-aged regions where at least 20 percent of the population is above 65 years old. However, there is only one district hospital in these areas or the patients need to spend at least 40 minutes driving their car to other hospitals in the urban areas. It is difficult for the patients with terminal disease to accept hospice care at his home. We have established a hospice team for hospice home care and hospice hospitalization in Cishan Hospital since 2015. We use SWOT analysis to identify strengths, weaknesses, opportunities, and threats of this plan, and try to find the strategies to overcome the difficulties
