輕度認知功能障礙及健康年長者語意流暢度測驗表現與大腦白質的關聯研究

Association between Category Fluency and White Matter Microstructure in Older Adults with and without Mild Cognitive Impairment

摘要


過去研究指出輕度認知功能障礙者與阿茲海默氏症者皆有較差的語意記憶表現。本研究透過語意流暢度測驗的動物及水果類別,來探討輕度認知功能障礙者在數量上之語意記憶表現,並分析過去研究所提出在動物類別語意流暢度測驗中認知歷程之質性指標,進而以相似方法發展水果類別之質性指標,以了解在不同類別中表現的異同。研究同時結合腦部白質的關聯分析,並將語意流暢度測驗表現依時間區段作劃分,以了解輕度認知功能障礙者在不同指標中的動態變化。本研究募集經教育年數及性別配對的21位年輕人、21位認知正常年長者及24位年長者具有輕度認知功能障礙,並對其進行神經心理功能的評估及安排腦部磁振照影的資料蒐集。研究發現輕度認知功能障礙組的語意記憶表現及腦部白質完整度明顯較其他兩組差,特別是在兩類別之正確反應的總數及水果類別之前30秒區段所產出的正確反應,及腦部白質纖維指標在左側下額枕束、胼胝體顳葉頂端及海馬的位置。本研究指出在輕度認知功能障礙者中,類別語意流暢度測驗表現及相關腦部白質完整度下降,顯示在前驅期發展阿茲海默氏症的病程中即具有語意記憶相關功能的退化。

Background: Previous studies have demonstrated semantic memory impairments in individuals with Alzheimer’s disease (AD) and mild cognitive impairment (MCI). Objective: The present study aimed to examine both the quantitative and qualitative perspectives of the performances in animal and fruit category fluency tasks across time, and investigated the dynamic relationship between the indices of category fluency tasks and the integrity of white matter microstructures in MCI. Methods: Three education- and gender-matched groups participated in this study, including 21 young adults, 21 cognitively normal older adults (CN), and 24 MCI participants. All participants underwent a comprehensive neuropsychological battery and received structural magnetic resonance imaging. Results: The results showed that the MCI group showed worst performance in category verbal fluency compared to the CN and young groups, particularly in the words generated at the first 30 seconds of the fruit fluency task and the number of total correct responses of both categories. Moreover, the MCI group revealed the lower mean GFA values of left inferior fronto-occipital fasciculus (IFOF), corpus callosum (CC) to temporal pole, and hippocampus parts compared to the CN and young groups. Conclusion: This study indicated that the declines of category fluency performances and white matter integrity revealed the early semantic and neuropathological changes in prodromal AD.
