醫院權屬別與健保相對非獲利醫療服務提供之關係

摘要

本研究旨在探討健保中醫院之權屬別對健保相對非獲利醫療服務之提供，特別是在門急診與社區服務部分之差異。營利型私立及社團法人醫院，大多經營獲利醫療項目，社會期望公立醫院或其他以社會公益為設立宗旨之非營利醫院，應經營非獲利項目，但是主管機關卻未對公立及非營利醫院加以適當監督，因此，此類醫院是否以非獲利醫療與社區健康促進為宗旨並不明確。本研究目的為：探討不同權屬別醫院如非營利財團或其他法人醫院，在相對非獲利醫療項目與社區醫療服務之差異；以及探討公立醫院與非公立醫院，在相對非獲利醫療項目與社區醫療服務之差異。本研究材料與方法係使用由國家衛生研究院發行的2010年全民健康保險學術研究資料庫的次級資料，研究對象為期間使用特定醫療服務的病患，為瞭解健保申報資料中，醫院權屬別與醫療費用間的相關性與社區醫療服務之差異，本研究分別控制醫院層級、區域及是否為教學醫院、住民可支配所得及失業率狀況等變項，以複迴歸分析法進行檢驗。研究結果顯示：（1）財團法人醫院與私立醫院之非獲利醫療與社區服務之金額與費用占率，幾乎均高於公立醫院。（2）宗教法人醫院之精神疾病與社區復健服務方面高於其他權屬別之醫院（3）公立醫院在安寧居家療護項目較佳。

This study aims to explore the differences in relative unprofitable medical services in National Health Insurance by hospital ownerships especially in OPD/ER and community services. The private hospitals provide profit medical services mostly, we expect public and non-for–profit hospitals should provide unprofitable medical services mostly. But the government did not supervise public and non-for-profit hospitals well. Thus did these hospitals meet their non-for-profit vision or community benefit services were still unknown.Purpose of this study were to measure the differences of relative unprofitable medical services and community benefit services among non-for-profit hospitals and the differences between public and private hospitals. The panel data of 2010 NHI Registration database files of Taiwan were used for analysis with several variables controlled.
The results indicate that: (1) the performance and spending on community benefit services of non-for-profit hospitals and private hospitals are almost higher than public hospitals; (2) the performance and spending on community psychiatric services in religious hospitals are higher than other hospital ownerships; (3) the performance and spending on home hospice services in public hospitals are higher than other hospital ownerships.
