抗病毒藥物治療、Ｂ型肝炎病毒聚合脢基因序列變異、和肝細胞癌存活之初步分析

摘要

背景/目的: 抗病毒藥物治療降低B型肝炎病人體內的HBV病毒量，然而使用抗病毒藥物會導致HBV病毒polymerase基因上YMDD區域的突變，進而產生抗藥性。本研究目的在於初探抗病毒藥物治療以及HBV polymerase上的序列變異對於HCC病人存活狀況的影響。

方法: 我們從兩家醫學中心納入919名具有抗病毒藥物治療問卷的HCC病人，HCC相關的臨床狀況來自初始病歷資料，利用全國癌症及死亡登記檔連結，獲得研究個案死因和存活狀態資料。HBV polymerase之定序涵括兩個區域，Reverse Transcriptase基因區與RNase H基因區。使用Cox’s model和Kaplan-Meier存活分析探討各因子對HCC存活之影響。

結果: 4.6% (42/919)的研究個案曾接受過抗病毒藥物治療，其3年的存活率為84.6%，高於未接受過抗病毒藥物治療的43.9%(P=0.0013)，利用多變項分析，在控制人口學特徵和臨床因子後，使用抗病毒藥物治療仍舊傾向較好的預後，Hazard ratio為0.55(95%信賴區間:0.32-0.93；p=0.0270)。隨機選取35名病例定序HBV polymerase基因區，發現92個核苷酸位置之變異型佔20%以上，其中29個位置其不同核苷酸型態的兩年存活率相差10%以上，對這些核甘酸位置和病毒基因型進行主成份分析，發現病毒基因型和其相關的變異型之主成份可解釋41%的總變異量，然而另有3個與基因型無關的主成份仍能解釋約33%的變異。結論: 抗病毒藥物可能降低HCC病人的死亡率。HBV polymerase序列變異是否會影響病人存活值得較大規模的定序分析進一步探討。
