模組化之疾病個案管理資訊系統架構

摘要


個案管理是以病患為中心,由醫療人員組成的團隊共同參與病患照護,以加強病人衛教及管理師介入追蹤的方式,減緩疾病病程,提昇照護品質。個案管理師需要許多來自醫院電子病歷系統的資料,且工作內容包含大量的資料彙整處理,因此必須有支援個管作業的資訊系統。疾病種類繁多,不同性質的疾病的照護方式可能有很大差異,但針對不同疾病開發不同系統過於曠日廢時、不切實際,所以此研究目標為依各種疾病個管流程中相近之處提出模型,模組化個案管理資訊系統,以應用到各種不同的疾病個管,且各模組能依需要更換以滿足各種個管作業需求,節省系統開發時間。目前已依此模型開發HIV個管及糖尿病個管等兩個有相當差異之個管作業資訊系統,符合各自所需之作業流程,並有效減少系統開發所需時間。

Case Management is a patient-centered delivery of healthcare, carried out by a team of medical staffs, by emphasizing patient health education and proactive intervention and tracking of patients’ conditions, to control disease progression and improve the quality of healthcare. The work requires various data from the hospital information system, and includes a large amount of data processing and compilation, thus it is imperative to build a system that supports case management operations. While there exist a wide range of diseases, care for different types of diseases may varies from one to another. The aim of this study is to propose, by exposing the similarities of case management workflow, a model of information system, which may be applied to various types of disease case management and fitting the requirements by some replacement of its modules, and also saves the time of development. An HIV and a diabetes case management system were built with this approach. While the two have a wide range of diversity, the model has successfully applied to the system development of them, and resulted in reduced effort spent in the development process.
