以根尖牙周炎作為口腔局部感染指標與頸部動脈鈣化相關探討

摘要


背景介紹 此研究將探討牙周局部感染指標與頸部動脈鈣化的關聯。 方法: 此觀察性研究於收集於2011年9月15日-至2012年9月15日台大醫院牙科看診之病人接受全口放射影像攝影(PR)共(2106人次)挑選有台大醫院就診之病歷(642人占30.4%)。以環口放射線攝影檢視:齒槽骨破壞程度、缺齒數、根尖牙周炎、齲齒指數(DMF index)與頸部動脈鈣化之關聯。 結果 :於全口放射片影像上,齒槽骨破壞程度中度比輕微於頸部動脈鈣化之OR值為0.48(95%信賴區間 0.27~0.85),重度比輕微於得到頸部動脈鈣化上OR值為0.50(95%信賴區間 0.15~1.63) P值=0.039。 結論:臺大醫院病人有口腔感染狀態會增加頸部動脈鈣化的風險,齒槽骨破壞程度輕微,落齒數越高及有根尖牙周炎者,於全口放射影像中發現頸部動脈鈣化風險越高。

INTRODUCTION: This study aim to investigate the relationships between the severity of periodontal disease , tooth status on carotid artery calcification. METHODS: Based on a cross-sectional study, a total of 2106 dental panoramic graphs (PR)s was selected from National Taiwan University Hospital from sept.15th 2011 to sept.15th 2012. Dental panoramic radiographs (PR) were used to exam the alveolar bone loss , Tooth loss , and apical peritonitis ,Decayed, Missing, And Filled Teeth (DMF index), were used to define the severity of periodontal disease. RESULTS:IN examine paranoiac radiography, the Moderate degree of Alveolar Bone Loss compare with slight degree’s had carotid artery calcification, The Odds Raito is about 0.48 (95% CI 0.27 to 0.85), the Severe degree compare with slightly degree had crotid artery calcification ,the Odds Raito is 0.50 (95% CI 0.15 ~ 1.63) P value = 0.039. CONCLUSIONS: In NTUH patients, with oral infection (with slightly Alveolar Bone Loss ,Tooth loss and apical peritonitis ) had high risk presenting carotid artery calcification in examine PR.
