柑橘多酚對口腔潰瘍及口腔癌細胞影響之探討

摘要


臨床上,許多因素可以造成口腔潰瘍的發生,其中牙周疾病以及口腔癌也是其中兩種可能的原因,這指出了牙周疾病以及口腔癌對於口腔潰瘍的高度相關性。有許多來自於植物與水果中的多酚類物質已被証實了具有各種不同的藥理功能。因此,本研究的目的希望能夠評估柑橘多酚在牙科醫學上之應用。本研究中,柑橘多酚被開發用於改善纖維母細胞傷口癒合之能力以及用於抑制口腔癌細胞之生長。由我們的結果可以顯示,在低濃度下,柑橘多酚對於口腔潰瘍具有一增強再生能力之功能,此外,在高濃度下,柑橘多酚對於口腔癌細胞具有抑制生長之能力。因此本研究証實多酚可作為一漱口水之添加物質並應用於口腔潰瘍及其他口腔疾病。 此外,由於口腔疾病被治癒後,往往有時會造成患者牙周組織之齒槽骨缺損,而引導骨再生術在牙科醫學上是目前廣泛被用於修補齒槽骨缺損的一個方法。然而此方法最常見的問題即是需要一個穩定的薄膜作為屏障以避免滑動和在手術過程中會造成一新的傷口,這樣的問題均會造成患者的不便。為了解決此問題,本研究另一目的為發展出一創新的微型注射器,使之能夠直接帶有骨植入物並以注射之方式來重建由牙周疾病所造成的骨缺損,希望用於取代傳統的引導骨再生術。 本論文最大的貢獻於我們証實了柑橘多酚於牙科醫學應用上可作為一添加物用於口腔潰瘍癒合及抑制口腔癌細胞生長與爬行能力,以及一新微型注射器帶有骨植入物用於牙周疾病治療後所造成骨缺損之可行性。

In clinical, many factors could cause the oral ulcer occurring, including the two major factors ─ periodontal disease and oral cancer. It indicated that oral ulcer is high correlated with periodontal disease and oral cancer. Various polyphenolic compounds from plant or fruit are confirmed that they have different pharmaceutical functions. The purposes of this study were to evaluate the effects of citrus polyphenol (CP) on dental applications. In our study, the culture medium with CP was developed to improve the wound healing of fibroblasts and inhibit the growth of oral cancer cell. Our results show that, in the low concentration, CP will have an enhancing regenerative function for oral ulcer, and high concentration CP also shows a growth-inhibiting function for oral cancer cell. Therefore, the CP would be used to be a supplemental compound in mouthwash for oral ulcers and periodontal diseases. In addition, after curing the periodontal diseases, bone defect will happen on some patient. Guide bone regeneration (GBR) was widely used method for repairing bone lost from periodontal disease. But, there were some disadvantages exhibiting in the GBR method such as needing a stable barrier membrane and a new creative cavity during surgical process. To solve these disadvantages, the other purpose of this study was to develop the novel micro-injector, which is designed to carry bone graft substitutes to restore bone defects for bone regeneration in periodontal diseases. We hope the device would be used to replace the GBR method. The major contributions of our thesis are that we confirmed the feasibility of CP to be a supplemental compound for topical application onto the healing of oral ulcer, inhibiting proliferation and migration of oral cancer cell, and the feasibility of this novel micro-injector carried bone graft substitutes to restore the bone defects of periodontal tissue in dental application.
