乾癬病患之化學激素濃度與疾病嚴重度相關性

摘要


背景:浸潤到表皮的嗜中性球在乾癬的生成上扮演最重要角色。化學激素則負責嗜中性球移動的工作,並與疾病的嚴重度有關。本實驗是想要研究疾病嚴重度與化學激素的相關性。目的:研究可利用的化學激素指標,是否用來評估乾癬的嚴重度。方法:於本院收集16位乾癬病患,測量血液、尿液、及唾液中化學激素IP-10 (interferon inducible protein 10), MCP-1 (monocyte chemoattractant protein-1), MIG (monokine induced by IFNγ), RANTES (regulated upon activation normal T cell expressed and secreted), IL-8 (interleukin 8)的濃度,並於一個月後重複採集標本;同時收集50名健康成人體液標本為對照組,利用免疫螢光流式細胞儀測定。結果:乾癬病患血清中化學激素IP-10, MIG 濃度較正常對照組高出許多(p<0.0001),並與PASI 呈現正相關 (Spearman’s γ=0.91)。而乾癬病患血清中化學激素RANTES, IL-8卻較對照組為低(p<0.05)。尿液及唾液化學激素則在各組之間則沒有太大差異。結論:血清中IP-10濃度與乾癬病患PASI分數呈現良好正相關,可考慮作為臨床研究上替代PASI評估乾癬疾病嚴重度的工具。

Background: Infiltrating neutrophils play a key role in pathogenesis of psoriasis. Chemokines are involved in leukocyte homing and contribute to inflammatory process in psoriatic skin lesions. Objective: To investigate the possible chemokine index for the assessment of the disease severity. Methods: Serum concentrations of chemokines IP-10, MCP-1, MIG, RANTES, and IL-8 were assayed in 16 psoriasis patients with serial sampling and 50 healthy controls by immunofluorescence flow cytometry with beads array. Results: Serum IP-10, MIG concentrations in psoriasis patients were higher than in controls (p<0.0001), and correlated significantly with PASI score. While serum RANTES, IL-8 in psoriasis patients were lower than in controls (p<0.05). No significant differences in urine and saliva chemokines concentrations among all groups. Conclusions: Serum IP-10 levels fits well with PASI score in psoriasis patients, even under different treatment regimen. It may be used as parallel parameter for PASI score in researching of psoriasis.
