國立台灣大學附設動物醫院1999年至2003年犬隻腫瘤病例之回溯性調查研究

摘要


癌症已成為近年來伴侶動物主要死亡原因的重要疾病之ㄧ,而癌症流行病學研究不只在探索可能致癌相關的危險因子及環境暴露極具價值之外,對於寵物狗自然發生的腫瘤疾病作為人類癌症的臨床治療模式與病因研究參考的角色也日益受到重視。但是,在台灣,對於犬隻腫瘤疾病之流行病學調查仍付之闕如,其迫切之需要性是顯而易見的。此研究是利用1999年到2003年於國立台灣大學附設動物醫院初就診之犬隻病歷做醫院族群為主的回溯性調查,主要目的在於獲得此教學醫院族群中犬隻腫瘤之盛行率、腫瘤發生位置及組織型態之分布頻率、以及關於腫瘤病例之基本資料分析。研究結果總共收集到687個腫瘤病例並進行分析,在此五年間於台灣大學附設動物醫院的總盛行率為3.7%,其中有60%為惡性腫瘤。根據發生位置的分析,於乳腺及皮膚包含皮下組織有最高之發生頻率,佔所有腫瘤之66%,其次為造血系統、泌尿生殖系統、口腔咽喉系統,而根據組織型態之分類,源自上皮組織的頻率高於間質組織及造血組織。經由腫瘤發生位置及組織型態之综合分析而獲得在此族群前十名常見之腫瘤分布,依序為乳腺腫瘤、淋巴瘤、圍肛腺瘤、口腔腫瘤、軟組織肉瘤、睪丸腫瘤、肥大細胞瘤、脂肪瘤、傳染性性器腫瘤、鱗狀上皮細胞瘤。然而,此研究之結果也顯示了在台灣犬隻腫瘤發生情況及品種分布與其他歐美國家研究調查之差異性。此研究之最終目標希望能建立台灣地區犬隻腫瘤之資料庫,並藉由統計分析探索可能的致癌相關危險因子。

Cancer has become the major cause of companion animal death in recent years. Not only used to investigate possible risk factor and environmental exposure, cancer epidemiological study of the pet dogs with spontaneously developing cancer had also provided an excellent opportunity to study many aspects of human cancer from etiology to treatment. The urgency and demand of canine neoplasm survey was required in order to compare the impact of cancer for canine population in Taiwan. A retrospective hospital-based study was performed using data retrieved from medical records for the period January 1, 1999 to December 31, 2003 in the National Taiwan University Veterinary Hospital (NTUVH). The aims of this study were to acquire canine cancer prevalence, distribution and frequency of tumor sites and types, and background signalment information of neoplasm cases from the total hospital population. A total 687 neoplasm cases was obtained and analyzed. The overall prevalence rate of canine neoplasm in NTUVH for the 5-year period was 3.7%, among which 60% were malignant. The high prevalence of neoplasm developed in mammary gland and skin/subcutaneous tissue was observed which comprised 66% of all cases, followed by hematopoietic, urogenital, and oropharyngeal tumors. In terms of tissue of origin, epithelial tumors were more common than mesenchymal and hematopoietic tumors. The ten most prevalent canine neoplasm observed were presented in the order of mammary gland tumor, lymphoma, perianal gland tumor, oral tumor, soft tissue sarcoma, testicular tumor, cutaneous mast cell tumor, lipoma, transmissible venereal tumor, cutaneous squamous cell carcinoma. The results of this survey revealed considerable variation of neoplasm prevalence and breed distribution in Taiwan when compared with other areas. The ultimate goal of this study is to establish a database of canine cancer in Taiwan and investigate potential risk factors by statistical analysis.
