社區藥局藥師在長照體系的角色與功能之探討

A Study on the Role and Function of the Pharmacist in the Community Pharmacy in Long-term Care System

摘要

老年人在家養老的同時，面臨諸多用藥安全上的風險問題。而長照體系未見老年人用藥照顧的需求服務，形成照顧上的死角。如何提供老年人最適用藥的照顧機制，引發本論文探討的動機。
本研究的分析結果，認為社區藥局藥師進入長照體系，可以實現走到巷口就有服務，對於老年人的照顧專業化及服務普及化，是有正向的影響。
最後結論統整提出建議，一則建議應使照顧專業化；二則建議利用社區藥局充實服務提供點；三則建議將社區藥局藥師納入長照體系常規的服務；許老年人一個有品質有尊嚴的生活。

The elderly face a lot of drug safety issues while pension at home. But the long-time care system did not see the needs of elderly medication-care services, resulting in care of the dead ends. How to provide the most suitable drug care in the elderly care mechanism? It is to induce the motivation of this paper.
Analysis of the results of this study, the community pharmacies and pharmacists entered the long-term care system, can be achieved willing to go to the alley there is a service. There is a positive impact on the universal care of the high quality for the elderly.
Finally, make three suggestions: First, the proposal should make care professional. Second, the proposal should make use of the community pharmacies to enrich the service delivery points. Third, suggested that the community pharmacists should be included in the routine services of the long-term care system. Hope to promise a quality of life and dignity of the elderly.
