血液透析中心屬性、委外考慮因素與委外決策之關係

摘要


企業經營所要強調的是如何降低成本,提高競爭優勢。如果各個企業仍維持舊有的工作型態而不從事改變,則無法滿足現今時代的需求。血液透析中心運用外部資源來提昇自己的競爭優勢已成為組織運作不可避免的現象。 本研究之目的在探討:(1)血液透析中心委外經營現況,(2)血液透析中心基本屬性與其委外經營決策之關係,(3)血液透析中心委外考量因素與其委外決策之關係。本研究依研究目的,將研究架構分成委外經營現況、委外考量因素及委外決策三大構面,建立了研究假設、變數操作性定義與衡量,並對血液透析中心發放問卷以獲得結論。 經由統計分析結果,本研究發現:(1)不同屬性之血液透析中心皆已普遍採取業務委外經營方式,並且考慮運用委外業務資源,以增進其優勢,(2)就委外項目而言,血液透析中心普遍以庶務為優先業務委外項目,再進而是醫療品管的委外,(3)由委外經營之血液透析中心「委外考量因素」與「委外決策」進行相關性分析,結果顯示目前委外考量因素會促進未來委外的決策,(4)在「委外考量因素」方面,不同屬性的透析中心,除了業務發展的考量因素達到統計上的顯著差異外,在其餘因素部分未達顯著性差異.(5)就不同規模透析中心之「委外考量因素」與「委外決策」互動情形而言,顯示皆具有正向的相關。 血液透析中心在規模效益與專業化的考量之下,透過委外業務資源,可減低自身內部的經營問題。未來在健保給付政策與同業競爭的壓力下,血液透析中心採用業務委外,逐漸已被視為解決經營問題的重要策略。 關鍵詞︰血液透析、委外、外包

The ultimate goal for business management is to elevate the competitiveness of the company through effective reduction of cost. No company would be able to survive in the business environment today without constantly adapting themselves to the changes in the business style. A proper utilization of external resources to elevate its competitiveness, therefore, has inevitably become an important part of the organizational management for Hemodialysis centers. The purpose of this study is to provide an analysis on: (1) the status of the existing outsourcing management by Hemodialysis centers. (2) The business nature of Hemodialysis centers and how it affects the decision-making for outsourcing. (3) Influencing factors of outsourcing management and how they affect the decision-making process for outsourcing. The study will be structured, based on its purpose, into three main phases, i.e., the current status of outsourcing management, its influencing factors and the future decision-making process for outsourcing. Conclusion has been reached through research methods including the establishment of research hypothesis, operational definition of variables and the assessment, as well as questionnaires distributed to the relevant Hemodialysis centers. The survey statistics and results of analysis showed that:- (1) Outsourcing management is a common practice adopted by Hemodialysis centers of different business nature, who consider the utilization of external business resources as an effective way to enhance their business strength. (2) In terms of the business items being outsourced, administrative routines are the prioritized items for outsourcing, followed by tasks relating to quality control of medical service. (3) A correlation analysis has been conducted between the decision-making for outsourcing service and the influencing factors of outsourcing management. The result was that, the influencing factors have positive impact on facilitating the decision-making for outsourcing. (4) In terms of the influencing factors, it has been found that, among the Hemodialysis centers with different business natures, significant variation only occurred in the case of the “business development” factor. No significant variation has been identified in other factors. (5) Despite the varied business scope of Hemodialysis centers, statistics show that a positive correlation exists between the influencing factors and the decision-making for outsourcing in all cases. Considering the scale effectiveness and professionalism in the management of Hemodialysis centers, outsourcing is an effective approach to reduce the internal management problems. With the increasing competition from the industry and pressure from the National Health Insurance Payment Policies, outsourcing management has gradually been recognized as an important strategy in dealing with the various management issues with the Hemodialysis centers. key word:Hemodialysis;outsourcing
