基質細胞衍生因子-1及其受體CXCR4之基因多型性與口腔癌的相關性探討

摘要


口腔癌是台灣十大癌症發生率及死亡率的第6名,而且超過85~90%都是鱗狀細胞癌。此外,每年有超過2400個因檳榔的普遍使用而被診斷為口腔癌的新病例,成為目前台灣發生率及死亡率偏高的癌症。基質細胞衍生因子-1 (SDF-1)是趨化因子之一,對血球有很強的趨化作用,與其受體CXCR4有特異性的結合。最近有許多研究發現SDF-1與CXCR4與腫瘤生長、增生、轉移與血管新生有關,且其基因多型性也與癌症的發生有密切的相關性。因此,本實驗利用聚合酶鏈限制性連鎖反應-切割片段長度多型性(PCR-RFLP)的方式,分析113位口腔癌病人與284位非癌症之對照組,其SDF-1與CXCR4基因多型性的相關性。結果發現在SDF-1基因上帶有A/G基因型的個體會比帶有G/G基因型的個體有1.86倍罹患口腔癌的機率。另外,當口腔癌病人在CXCR-4基因上帶有至少一個T基因型的個體,其罹患口腔癌後會有2.66倍的機率進展為癌症第三或第四期。因此,我們發現,在台灣的個體中,SDF-1基因上帶有A/G基因型會增加其罹患口腔癌機率,而CXCR-4基因上帶有至少一個T基因型的個體其罹癌後會有較高的機率轉變成末期。因此,希望藉由此研究可以輔助口腔癌患者其預後的診斷。

Oral cancer is the sixth leading cause of cancer death in Taiwan, and more than 85~90 % of cases are oral squamous cell carcinoma (OSCC). The aim of this study was to evaluate the relations of SDF-1 and its receptor, CXCR4, gene variants on oral cancer risk. PCR-RFLP(polymerase chain reaction- restriction fragment length polymorphism) was used to measure SDF-1-3’A and CXCR4 gene polymorphisms in 284 controls and 113 oral cancer patients. After being adjusted for age, individuals with A/G heterozygotes of SDF-1 had a higher risk of 1.86 fold to develop oral cancer as compared to those with G/G wild type homozygotes. Furthermore, oral cancer patients with at least one T allele of CXCR4 gene had a risk of 2.66 folds to progress to stage III or IV. SDF1-3’A gene polymorphism may be considered as a factor of increased susceptibility to oral cancer, and at least one T allele of CXCR4 gene are associated with the development of stage III or IV and the induction of lymph-node metastasis of oral cancer disease in Taiwanese.
