濾鏡片誘發腦波訊號 P(100)對乾眼症患者之研究

摘要


研究目的: 本研究目的在於透過腦波儀量測及閃光刺激,所產生之P100誘發電位。在不配戴濾鏡及配戴四支濾鏡下,分析腦波P100的變化,預測乾眼症患者在配戴濾鏡下對畏光的改善能力。 研究方法: 本研究是使用灰色及玫瑰色兩種不同色系的濾光鏡片;包括,應用於乾眼症病患之腦波P100(Lambd)誘發研究。其中灰色為單一穿透度,玫瑰色共使用三種不同的穿透度所製作的濾光鏡;實驗進行中,我們讓患者在腦波儀(Electroencephalogram)的監測下及透過閃光刺激,在不配戴濾鏡及配戴四種濾鏡下,分析腦波P100的變化。藉由濾鏡對應於腦波P100的變化反應,預測乾眼症患者在配戴濾鏡下的情形。本實驗參與的患者,皆接受眼科裂隙燈眼睛疾病檢查及Shimer淚液評估測試;淚液試紙上的淚液長度小於10mm為輕度乾眼,小於5mm為重度乾眼。女性25位、男性6位,其中25位有嚴重乾眼症狀及6位輕度乾眼,平均年齡約50歲(20~79歲)。使用迴歸模型計算出來的β值代表模型預測能力,並利用顯著性 F test 檢定樣本,觀察應變數 Y 和所有自變數Xn 之間的相關性及相依樣本t考驗分析。 實驗結果: 實驗結果顯示,預測乾眼症患者配戴RL1、RL2、RL3、GL濾鏡後對畏光症狀有顯著的改善(P<0.05),濾光鏡片可用於降低強光帶來的不舒適感,並且降低UV光以及特定波長之可見光,以及減緩乾眼症患者不適的症狀。配戴與未配戴濾鏡對腦區,顳葉區,枕葉區、額葉區的腦波檢查現正向反應,表示濾鏡對於不同腦區P100的反應有正向增強的效果,也說明可以預測乾眼症患者經由濾鏡可以改善畏光的能力。

Two filters including the colors of gray and rose used as assistive devices to improve photophobia symptom for patients of Keratoconjunctivitis sicca with benign essential blepharospasm have been demonstrated in this thesis. The EEG P100 (Lambda) evoked potential was analyzed through optical stimulus by filters. Filters with characterized penetration coefficients were designed for clinical experiments approved by National Army Force General Hospital IRA committee. By wearing four different characterized filters, P100 VEP for the subjects were measured and analyzed via IBMSPSS 22 software. 31 subjects were recommended from Doctors diagnosed to be Keratoconjunctivitis sicca with benignant essential blepharospasm. Six of them were classified as control group. Average age of the subjects was 50 years old. Subjects receiving ophthalmic slit lamp examinations and ocular surface disease Shimer tear assessment tests were performed. Time for the experimental under natural light without wearing glasses were limited in three minutes. Wearing with pale pink color depth was also observed for 3 minutes. The experimental result has shown the correlations among all the filters and Keratoconjunctivitis sicca with benignant essential blepharospasm. EEG P100 is suggested as a good marker for the investigation.
