探析臺灣醫師繼續教育提供者之意見與態度

摘要


研究目的:本研究旨在研究實施「醫師繼續教育」制度下,探討CME(Continuing Medical Education CME)提供者辦理「醫師繼續教育」各構面選擇之意向,並對辦理「醫師繼續教育」時整體成效的滿意度;同時了解醫學繼續教育提供者在籌劃,設計,辦理時與各構面間的相互影響,以提供未來探討辦理「醫師繼續教育」之參考。 研究方法及資料:本研究工具採結構式問卷,問卷內容中各變項由專家座談會及參考文獻形成,總計教學醫院119家,醫師公會25家,醫學會26家,共170家,每家以寄發兩份問卷,本研究寄發340份問卷,問卷有效回收計185 份,回收率54.41﹪。本研究經資料蒐集完成之後,經鍵入、譯碼、核對,並去除無效問卷後,以敘述性分析及推理性分析呈現,分析方法以SPSS for windows 12.0版套裝軟體進行「信效度分析」、「變異數分析」及「卡方檢定」。 研究結果:調查醫師繼續教育提供者之間各構面結果,顯示師資,教育內容與主題,教育成效及辦理合適時段構面具顯著性的差異,其餘的構面則具有較一致的看法。而提供者以不同機構、年資、職稱或辦理頻率來分析其滿意度結果皆高於71.18。研究顯示提供者不但對繼續教育有較高的滿意度而且重視醫療政策之影響,因而醫療政策或生態仍影響辦理醫師繼續教育之方向與內容。 結論與建議:醫師繼續教育之政策方針與醫師法之繼續教育規範,容易造成參加繼續教育只為積分累積與定期換照的目的,教育提供者應藉由課程的整理安排達到醫師自我學習的目的,避免醫師繼續教育的意義流於形式。而繼續教育提供者期待教育內容主題、師資的加強和多元化,能更合乎醫療政策及臨床醫學實務之需求,以彰顯教育成效。因此相關醫學團體如醫學院,教學醫院,醫學會,醫師公會,醫教會,衛生局等應落實辦理醫師繼續教育,以促進教育之品質及成效。 關鍵詞: 醫師繼續教育;教育提供者;滿意度;醫療政策

Objective:The objective of this study is to examine the intentions and satisfaction of CME providers and to understand the influence of CME planning, designing and conduct with other dimensions to provide references for future CME. Methods and Materials:A close-ended questionnaire was used in this study. All the options in the questionnaire were formulated on the basis of expert symposia and references. The 170 subjects in this study included 119 teaching hospitals, 25 Taiwan medical associations and 26 Taiwan societies of medicine. Of the 340 distributed questionnaires, 2 questionnaires per subject were issued, and 185 were collected and viewed as valid. The overall valid response rate was 54.41%. After the data collection, valid samples were processed and analyzed by descriptive statistics, Chi-square tests and ANOVA by SPSS for windows 12.0. Results:The result of every dimension of the study on CME providers indicates that there are apparent differences between the faculty, the content and subjects of courses, and teaching effect. However, the other dimensions have a similar conclusion. The satisfaction of the CME providers with different organizations, seniority, title or holding frequency are always higher than 71.18 over 100. The study also tells that providers are not only satisfied with CME to a higher degree but also concern much more on medical policy. Thus, medical policy and environment still have an effect on the direction and the content of the CME administration. Conclusion and Suggestion:The policy of CME may result in doctors attending continuing medical education just for the purpose of periodically accumulating credits and renewing their licenses. Education providers should, therefore, strive for the goal for doctors to engage in self-learning by arranging courses and encouraging lifetime learning by strengthening learning motivation, and thus, preventing CME from drifting into formalism. However, CME providers who are actually engaged in the medical profession may expect the reinforcement and diversity of the teaching content, subjects and faculty, which may be matched to the demand of medical policy and clinical medical practice, and can also manifest the effect of education. Above all, the related medical organizations, such as medical colleges, teaching hospitals, medical associations, the committee of medical science education, and the department of health should fulfill CME in order to achieve educational goals and promote the quality and effect of continuing medical education.
