紀錄久坐工作者的光體積劑量儀與心電圖訊號進行心率變異度分析與比較

A comparison of photoplethysmography and ECG recording to analyse heart rate variability in male workers

摘要


目的: 根據過去文獻所進行的分析心率變異度(Heart rate variability, HRV)研究,研究進行的方向多為量測心電圖(Electrocardiography, ECG)的R-R波間期(R-R interval)並對於時域部分以及頻域部分進行各別的分析,且由分析之結果評估自主神經系統功能,目前心律變異度的主流儀器仍為心電分析儀,但心電分析儀在訊號之紀錄仍有技術上需克服之問題,而現今醫療經常使用的光體積計量儀(photoplethysmogram, PPG)同樣能進行心率變異度的分析,因此本研究希望探討光體積計量儀連續監控周邊微血管的血液變化獲取心跳週期的脈波波形與心電圖得到的電訊號頻譜波形兩者所分析得出的心率變異度指標進行比較並與受試者本身狀況進行討論。 材料與方法: 本研究自民國99年1月至民國100年12月招募受試者610位,在受試者同意協助研究進行並簽屬同意書後,安排受試者進行睡眠生理檢查,並透過自行開發之訊號分析程式分析睡眠生理檢查之訊號,根據研究目標進行特定族群之篩選出444位受試者,分別依據阻塞型睡眠呼吸中止症(Obstructive sleep apnea, OSA)有無以及血壓高低程度兩種模式進行分組,統計分析使用皮爾森相關係數對於兩種量測結果分析所得之心率變異度指標探討相同指標結果的相關性,使用單因子變異數分析探討不同程度間心率變異度指標之差異,使用成對樣本T檢定探討睡眠期別間Awake期以及睡眠期別N2期等心率變異度指標之差異。 結果: OSA的有無以及血壓高低程度分組在心率變異度的各項指標經過單因子變異數分析結果發現各組間的生理基本資料的指標與睡眠檢查資料的指標上有顯著的差異,而在皮爾森相關係數分析發現在心率變異度分析上睡眠期別N2期別比睡眠期別Awake期別較多的指標達到顯著,最後在成對樣本T檢定不同分組的受試者睡眠期別Awake期與睡眠期別N2的心率變異度變化發現隨著受試者的族群的不同,經過統計分析結果差異性達顯著性指標也不相同。 結論: 根據本研究結果可能顯示人體在睡眠期別的N2期心臟的跳動與血管的脈動有較佳的相關性,因其睡眠狀態中人體受到的干擾因子較醒著的時候少,而不同的睡眠期別之心率變異度的差異會依照受試者族群的不同而有所差異。

Objective: The signal is recorded by Electrocardiography(ECG) can measures of heart rate variability (HRV) are used to assess autonomic nervous system function. Photoplethysmography (PPG) is continuous monitoring of changes in blood volume in a portion of the peripheral microvasculature measures pulse waveforms. We investigated the applicability of using photoplethysmography to analyse heart rate variability by using the same analytic process to photoplethysmography and electrocardiography recordings made simultaneously. Material and Methods: 610 male workers recruited subjects undergoing the sleeping examination during July 2007 to March 2010.According the results by screening out of 444 subjects who have completed sleep stages to become the validated objects of analysis,and used the signals analysis program with photoplethysmography and electrocardiography signal data to analyze each person’s heart rate variability. According to the severity of apnea hypopnea index and blood pressure to divide groups.In this we divided our paticipants to four groups. Results: In the different OSA severity and blood pressure, The heart rate variability by electrocardiography and photoplethysmography show more index in significant correlation on sleep stage N2 than sleep stage awake.In pair-t test our result show the significant differences heart rate variability index with different groups are different. Conclusions: For this study show the correlation on our heart rate variability index between photoplethysmography and electrocardiography, but some index present different result due to classification of subjects.
